Lung inflammation in the pathogenesis of rheumatoid arthritis

Abstract: The primary manifestation of rheumatoid arthritis (RA) is articular disease; however, extra‐articular disease can also occur. In particular, pulmonary disease is a leading cause of morbidity and mortality in individuals with RA. Herein, we will review the types, prevalence, risk factors, and potential pathophysiology of lung disease in individuals with established RA. We will also discuss the emerging understanding of potential role of the lung in the generation of RA‐related autoantibodies during a period of … Show more

“…Our findings suggest that early-life inhaled exposures such as passive cigarette smoking could predispose individuals to develop RA not explained by later personal smoking behaviors. These results add to the mucosal paradigm of RA pathogenesis (5)(6)(7), where inhalants in pulmonary mucosa may trigger biologic processes that contribute to RA-related autoantibody production years before clinical RA symptoms emerge. Personal smoking is one of the most well-established modifiable risk factors for incident RA, whereas the influence of passive smoking is less well understood (14).…”

“…Rheumatoid arthritis (RA) is a debilitating systemic inflammatory disease characterized by prominent polyarthritis with associated morbidity and mortality (1)(2)(3). In its pathogenesis, considered an interplay of genetic and environmental exposures (4), lung inflammation is strongly implicated as an initial site of immune dysregulation and RA-related autoantibody production (5)(6)(7). Thus, smoking, personal (active) and passive, has been of interest as a major modifiable environmental risk factor for seropositive RA, which is characterized…”

“…Pulmonary lesions, as well as vascular, airway, pleural and parenchymal diseases, appear in approximately 70% of patients with RA 23 . RA‐related pulmonary diseases involving the airways such as COPD, asthma, bronchiectasis, and cricoarytenoid arthritis, are known to be frequent, but interstitial lung disease (ILD), a parenchymal disease, is also common, and it has been reported that up to 58% of RA patients have ILD 24 .…”

“…These pulmonary diseases are caused by various genetic and environmental factors and usually share a common risk factor smoking. In addition, they have similar respiratory symptoms, and when the disease progresses, both obstructive and restrictive patterns appear simultaneously on PFT appear 23,26 . Therefore, in this study, pulmonary diseases other than COPD having mixed pattern on PFT may be included; further investigations such as radiological findings may be used to distinguish pulmonary diseases more accurately.…”

“…When one or more of these autoantibodies are detected in serum, RA is defined seropositive (RA+) and the disease might manifest with more severe symptoms such as erosive joint degeneration and rapid radiographic progression [1,6].…”
