Lumbosacral junction injury associated with unstable pelvic fracture: classification and diagnosis.

Abstract: Lumbosacral junction injury must be considered in patients with an unstable pelvic fracture, especially in those with an associated sacral fracture. CT is necessary because these conditions may be overlooked on radiographs, although outlet radiographs are better than CT scans for depicting the possibly associated L5-S1 intervertebral disk lesion.

“…True lateral radiographs are very useful for diagnosing this specific fracture (43). Lumbosacral junction injuries, which occur when sacral fracture lines pass cranially through or medial to the S1 articular process, are rare (45). The sacral plexus can be injured as a result of sacral fractures.…”

“…Fracture-dislocation with displacement results in substantial lumbar disc disruption and a decrease of load-bearing capability. Defects of the anterior column make decision-making more challenging [18][19][20]. Anterior column insufficiency in the acute phase is associated with sagittal balance distortion.…”

The decision for diagnosis, spinal cord injury detection, classification, and treatment selection imaging like CT scan and MRI have important roles[21].A valuable classification of lower lumbar fractures may contain: Isolated process fractures such as transverse process or spinous fractures; 2.…”
