Lumbale Spinalkanalstenose

Abstract: Lumbal spinal stenosis is gaining more and more clinical relevance because of changing population structure and increasing demand on lifequality in the elderly. Current treatment recommendations are based on clinical experience, expert opinions and single studies rather than on proven evidence. The radiologic degree of stenosis does not correlate with the patients' clinical situation. It is not the main factor indicating surgery but rather the typical history and spinal claudication. Symptomatic patients with … Show more

“…Patients with symptomatic degenerative central LSS with relevant spinal claudication who did not improve after intensive conservative treatment in accordance with international guidelines and recommendations [21,25,34] for at least 3 months were included consecutively. Conservative multimodal treatment comprised physiotherapy (including flexion exercises and traction), physical therapies, strengthening exercises, back schools, braces, medication, infiltration therapies (e.g., epidural steroid injections), patient information about the characteristics and course of the disease, cognitive behavioural therapy and electrical stimulation therapy on an individual basis.…”

“…Symptomatic degenerative central lumbar spinal stenosis (LSS) is one of the most frequent indications for spinal decompressive surgery [25,29]. Spinal claudication is the main symptom in these patients, who are usually older [33,39].…”

“…Undercutting decompression is a standard procedure in the therapy of lumbar spinal stenosis [3,4,13,21,25]. Accompanying segmental instability may occur, which is caused either by degenerative segmental disease involving the intervertebral disc, the zygapophysial joints and ligaments, or as a consequence of the surgical decompression itself [7,8,15,17].…”

“…Diese Begriffe sind aber in Bezug auf ihre klinische Relevanz bislang nicht eindeutig validiert [6] Diese Empfehlung beruht auf der Untersuchung von asymptomatischen Individuen über 60 Jahren, bei welchen die MRT bei 57% pathologische Befunde zeigte. Bei 36% konnte ein Bandscheibenvorfall und bei 21 % eine SKS nachgewiesen werden [15].…”
