Lumbar Interspinous Bursitis (Baastrup Disease) in a Symptomatic Population

Abstract: Lumbar interspinous bursitis (Baastrup disease) is uncommon but not infrequent in symptomatic patients undergoing lumbar spine MRI. Patients with MRI evident Baastrup disease tend to be older, have central canal stenosis, bulging discs, and anterolisthesis. Further investigations determining the clinical significance of this finding are necessary.

“…Our observations are consistent with the notion that the mechanical back pain that is usually associated with Baastrup's disease is actually secondary to the degenerative spine disease that often accompanies Baastrup's disease, such as disc degeneration, spinal stenosis, and spondylolisthesis, etc. [2,7]. Indeed, Maes suggested [7] in 2008 that we should ''consider lumbar interspinous bursitis as Baastrup's sign instead of Baastrup's disease.…”

“…Impingement of spinous processes has also been recognized in man as Baastrup disease or kissing spines syndrome. In man the clinical significance remains unresolved (Maes 2008, Kwong 2010. Clinical signs of back-pain include reduced performance, ill-defined lameness (particularly in the pelvic limbs), bucking, resistance to saddling and sacro-iliac pain (Jeffcott 1980, Walmsley 2002, Dyson 2003, Landman 2004, Zimmerman 2012.…”
