Hypertrophy of facet joints is associated with a high risk of central lumbar spinal stenosis (CLSS). However, no research has reported the effect of inferior articular process hypertrophy in CLSS. We hypothesize that the inferior articular process’s cross-sectional area (IAPCSA) is larger in patients with CLSS compared to those without CLSS. Data on IAPCSA were obtained from 116 patients with CLSS. A total of 102 control subjects underwent lumbar spine magnetic resonance imaging (LS-MRI) as part of a routine medical examination. Axial T1-weighted images were obtained from the two groups. Using an imaging analysis system, we investigated the cross-sectional area of the inferior articular process. The average IAPCSA was 70.97 ± 13.02 mm2 in control subjects and 88.77 ± 18.52 mm2 in patients with CLSS. CLSS subjects had significantly greater levels of IAPCSA (p < 0.001) than controls. A receiver operating characteristic (ROC) curve was plotted to determine the validity of IAPCSA as a predictor of CLSS. The most suitable cut-off point of IAPCSA for predicting CLSS was 75.88 mm2, with a sensitivity of 71.6%, a specificity of 68.6%, and an area under the curve (AUC) of 0.78 (95% CI: 0.72–0.84). Greater IAPCSA levels were associated with a higher incidence of CLSS. These results demonstrate that IAPCSA is a useful morphological predictor in the evaluation of CLSS.

Актуальность. Боль в поперечно-крестцовом отделе позвоночника остается одной из наиболее актуальных медицинских и социальных проблем. Задачей исследования было оценить эффективность комбинированного использования трансфораминальной и каудальной эпидуральных блокад для лечения болевого синдрома пояснично-крестцового отдела позвоночника на фоне дегенеративно-дистрофических заболеваний. Материалы и методы. В исследование были включены 228 (85 мужчин, 143 женщины) больных с болью в пояснично-крестцовом отделе позвоночника в возрасте 52,07 ± 13,95 года, которым были проведены трансфораминальные эпидуральные блокады (n = 114) или каудальные эпидуральные блокады (n = 114). Результаты. Положительный эффект был зарегистрирован в 150 (65,8 %) случаях. У остальных 78 пациентов при восстановлении болевого синдрома до 5 и более баллов по визуальной аналоговой шкале были выполнены повторные процедуры общим количеством до четырех в течение 4–14 дней после первой блокады. Их разделили на три группы: в группе 1 пациентам повторно провели трансфораминальные эпидуральные блокады (n = 26), в группе 2 — каудальные эпидуральные блокады (n = 26), в группе 3 — комбинированное применение трансфораминальной и каудальной эпидуральных блокад (n = 26). В начале интервенции, через 1 час, 3 недели и 3 месяца во всех группах определяли интенсивность болевого синдрома, симптомы натяжения (проба Ласега), объем активных движений в поясничном отделе позвоночника и качество жизни (индекс инвалидности Освестри). Выводы. Установлено, что комбинированное использование трансфораминальной и каудальной эпидуральных блокад является наиболее эффективным методом лечения болевого синдрома в пояснично-крестцовом отделе позвоночника и связано с положительной динамикой качества жизни пациентов. Трансфораминальные или каудальные эпидуральные блокады были также эффективны, но только в краткосрочной перспективе.

The mechanism of low back and leg pain involves mixed neuropathic and nociceptive components. Spinal neuropathic pain is related to increased levels of inflammatory cytokines and disrupted and increased permeability of the blood–spinal cord barrier, originally composed of tight junctions of capillary endothelial cells surrounded by lamina. The phase angle (PA) estimates cell membrane integrity using bioelectrical impedance analysis. We evaluated the predictive value of the PA for analgesic efficacy in lumbosacral transforaminal block. We retrospectively collected data from 120 patients receiving transforaminal blocks for lumbosacral radicular pain and assessed the PA before and 5 min following the block. Responders (group R) and non-responders (group N) were defined by ≥50% and <50% pain reduction, respectively, on a numerical rating scale, 30 min following the block; clinical data and the PA were compared. Among the 109 included patients, 50 (45.9%) and 59 (54.1%) had ≥50% and <50% pain reduction, respectively. In group N, the PA change ratio showed 88.1% specificity, 32.0% sensitivity, and 62.4% accuracy; a ratio of <0.087 at 5 min following the block predicted non-response. A PA change ratio of <0.087 at 5 min following lumbar transforaminal blocks predicted non-responders with high specificity.
