Lucky number 13: Association between center-specific chest wall stabilization volumes and patient outcomes

Abstract: BACKGROUND:Chest wall stabilization (CWS) improves outcomes for patients with chest wall injury (CWI). We hypothesized that patients treated at centers with higher annual CWS volumes experience superior outcomes. METHODS:A retrospective study of adults with acute CWI undergoing surgical stabilization of rib or sternal fractures within the 2019 Trauma Quality Improvement Program database, excluding those with 24-hour mortality or any Abbreviated Injury Scale body region of six, was conducted. Hospitals were gro… Show more

“…While strong associations between SSRF and improved patient outcomes were seen in previous observational studies, results of randomized controlled trials have not been consistent. 7,8,[11][12][13][14]33 These discrepant results raise concern for residual confounding in observational studies, since patients anticipated to have a positive outcome may have been more likely to undergo SSRF. Our analysis' use of an instrumental variable analysis mitigates this concern as in this approach patients are putatively "pseudorandomized" to receive SSRF based on the tendencies of the hospital at which they are treated.…”
