Background With advances in supportive and risk-stratified therapy, the 5-year survival rate of acute lymphoblastic leukemia has reached 85.5%. The ALL-2006 treatment protocol was modified and renamed the ALL-2013 protocol, with dose and duration changes. Objective To compare outcomes of the ALL-2006 and ALL-2013 protocols, with regards to mortality, remission, relapse, and three-year survival rates. Methods This was retrospective cohort study. Subjects were acute lymphoblastic leukemia (ALL) patients treated from 2011 to 2018 in Mohamad Hoesin Hospital, Palembang, South Sumatera. The three-year survival rates, relapse, remission rates and comparison of ALL-2006 and ALL-2013 protocols were analyzed with Kaplan-Meier method. Results Mortality was significantly correlated with age at diagnosis <1 year and >10 years, hyperleukocytosis, and high-risk disease status. Patients aged 1 to 10 years, with leukocyte count <50,000/mm3 and standard-risk status had significantly higher likelihood of achieving remission. Mortality was not significantly different between the ALL-2006 protocol group [70.6%; mean survival 1,182.15 (SD 176.89) days] and the ALL-2013 protocol group [72.1%; mean survival 764.23 (SD 63.49) days]; (P=0.209). Remission was achieved in 39.2% of the ALL-2006 group and 33% of the ALL-2013 group (P>0.05). Relapse was also not significantly different between the two groups (ALL-2006: 29.4% vs. ALL-2013: 17.9%; P>0.05). Probability of death in the ALL-2006 group was 0.3 times lower than in the ALL-2013 group (P<0.05), while that of the high-risk group was 3 times higher. Remission was 2.19 times higher in those with leukocyte <50,000/mm3 compared to those with hyperleukocytosis. In addition, relapse was significantly more likely in high-risk patients (HR 2.96; 95%CI 1.22 to 7.19). Overall, the 3-year survival rate was 33%, with 41.7% in the ALL-2006 group and 30.7% in the ALL-2013 group. Conclusion Three-year survival rate of ALL-2006 protocol is higher than that of ALL-2013 protocol but is not statistically significant. Age at diagnosis <1 year and >10 years, hyperleukocytosis, and high-risk group are significantly correlated with higher mortality and lower remission rates. However, these three factors are not significantly different in terms of relapse.

Leukemia is a most common children malignancy. Chemotherapy has been very effective in the management of leukemias. Chemotherapy is a systemic therapy, therefore the chemotherapy drug is not only directly related to the cancer but also damaged the normal tissue. Physiological effect of chemotherapy is important to be inspected because each individual is unique so it has different physiological effect, so it will require different treatment. The aim of this research is to understand physiological effect of chemotherapy in children with leukemia. This research is quantitative research, used descriptive analitic. The sample of this research were 30 sample with total sampling. The instrument used was a questionnaire of physiological effect of chemotherapy. The results showed that the physiological effect of chemotherapy were children with alopecia of 24 (80%) , Nausea of 17 (56.7%), vomiting of 16 children (53.3%); Decreased appetite of 14 children (46.7%); oral ulceration of 19 children (63.3%);Diarrhea of 3 children (10%); Fatigue of 14 children (46.7%); increase in risk factors for infection consisting of fever of 16 children (53.3%), cough of 13 children (43.3%), and flu of 8 children (26.7%); Bruises of 8 children (26.7%), bleeding of 14 children (46.7%); and other side effects there are 14 children (40%). The conclusion of this research is the most common physiological effects of children undergoing chemotherapy are alopecia, oral ulceration, nausea and vomiting, and fever

Background High-risk acute lymphoblastic leukemia (ALL) is one of the most common childhood malignancies in Indonesia. Many factors can inhibit the induction of remission. Hematological parameters are usually not normal. Identification of corresponding factors is important to increase the likelihood of successful inductions.Objective To assess for associations between hematological parameters and induction of remission in children with acute lymphoblastic leukemia.Methods Data were collected from medical records of ALL patients hospitalized in the Pediatric Ward at Dr. Kariadi Hospital from May 2014 – May 2016. Dependent variables were hemoglobin, leukocytes, platelets, and absolute neutrophil count (ANC) levels; the independent variable was induction of remission.Results Out of 55 patients, 33 (60%) had anemia, 6 (10.9%) had leukocytosis, and 1 (1.8%) had hyperleukocytosis, whereas 9 (34.5%) had leukopenia and 29 (52,7%) had normal leukocyte levels. Thirty-one subjects (56.4%) had thrombocytopenia, 15 (27.3%) had thrombocytosis, and only 9 (16.4%) patients had normal platelet counts. There were 29 (52.7%) with absolute ANC > 500, whereas 26 (47.3%) had ANC level ≤ 500. Most patients (80%) experienced remission induction, while 20% did not. There were significant associations between ANC level and induction of remission (P=0.010) as well as between platelet level and induction of remission (P= 0.033). Regression logistic test revealed that ANC level ≤ 500 was associated with a 7-fold lower remission event compared to ANC level > 500 (RR 7.147; 95%CI 1.38 to 37.14).Conclusion Lower ANC level (≤ 500) was significantly associated with lower remission compared to higher ANC level (> 500).
