<title>Photodynamic therapy with 5-aminoolevulinic acid-induced porphyrins and DMSO/EDTA for basal cell carcinoma</title>

Abstract: 763 basal cell carcinomas (BCCs) in 122 patients were treated by photodynamic therapy by 5-aminolevulinic acid (ALA) in cream topically applied, either alone, in combination with dimethyl sulphoxide (DMSO) and ethylenediaminetetraacetic acid disodium salt (EDTA) or with DMSO as pretreatment. After 3 hours cream exposure 40-200 Joules/cm2 of 630 nm laser light was given. Fluorescence imaging of biopsies showed highly improved ALA penetration depth and doubled ALA-induced porphyrin production using DMSO/EDTA. Tr… Show more

“…Recurrences were observed with lesions located on the nose of the morpheiform type treated with 1.0 mg/kg BW Photofrin and a light fluence of 200-215 J/cm 2 [53]. Topical ALA at a concentration of 20% was also found to be effective in similar clinical studies [54][55][56][57][58][59].…”

“…3 The concept that PDT can selectively destroy tissue has found applicability in the treatment of malignant and benign tumors involving the skin, gastrointestinal (GI) tract, retroperitoneum, genital and urinary systems, chest, central nervous system, and eye. 4 -13 PDT shows considerable promise as a primary treatment modality for localized superficial tumors 4,5,14 and is a potentially important form of adjuvant treatment for cancers of the GI tract. 8,9 The advantages of PDT include the low incidence of complications, 15 tumor responsiveness uncompromised by previous radiation or chemotherapy, subsequent use of radiation and/or chemotherapy, and repetition in multiple successive sessions.…”

“…Svanberg et al [32] found that only 16 of 25 nodular BCC (64%) showed a complete remission after one single treatment, whereas 100% complete remission rate was achieved after one additional treatment. Adding a penetration enhancer, e. g. DMSO, to the ALA formulation also could improve clinical results of topical PDT for nodular lesions [33,34]. Besides ALA, there are few other sensitizers that have been used topically.…”
