Behavioural interventions that address cardiovascular risk factors such as physical inactivity and hypertension help reduce recurrence risk following a transient ischaemic attack (TIA) or “minor” stroke, but an optimal approach for providing secondary prevention is unclear. After developing an initial draft of an innovative manual for patients, aiming to promote secondary prevention following TIA or minor stroke, we aimed to explore views about its usability and acceptability amongst relevant stakeholders. We held three focus group discussions with 18 participants (people who had experienced a TIA or minor stroke (4), carers (1), health professionals (9), and researchers (4). Reflexive thematic analysis identified the following three inter-related themes: (1) relevant information and content, (2) accessibility of format and helpful structure, and (3) strategies to optimise use and implementation in practice. Information about stroke, medication, diet, physical activity, and fatigue symptoms was valued. Easily accessed advice and practical tips were considered to provide support and reassurance and promote self-evaluation of lifestyle behaviours. Suggested refinements of the manual’s design highlighted the importance of simplifying information and providing reassurance for patients early after a TIA or minor stroke. Information about fatigue, physical activity, and supporting goal setting was viewed as a key component of this novel secondary prevention initiative.

Objective: Cognitive impairment usually occurs in the acute phase after stroke, but most stroke survivors experience some form of long-term cognitive deficit. The aim of this study was to establish the cutoff point of the Montreal Cognitive Assessment (MoCA-Beijing) in screening for cognitive impairment (CI) at 6 months of ischemic stroke or transient ischemic attack (TIA). Methods: A total of 301 stroke patients and 15 TIA patients were recruited. Patients were assessed at six months by the MoCA-Beijing and a formal neuropsychological battery. The 1.5 SD below the level of the norm on several tests indicated cognitive impairment (CI). Results: Most stroke and TIA patients were in their 60s (61.23 ± 10.60 years old). The optimal cutoff point for MoCA-Beijing in discriminating patients with CI from those with no cognitive impairment (NCI) was 24/25 (sensitivity 63.28%, specificity 71.22%, PPV = 73.68%, NPV = 60.37%, classification accuracy = 66.72%). The predominant cognitive deficits were visuospatial ability (84.85%), and then attention/executive function (79.27%). Conclusion: The MoCA-Beijing cutoff score for differentiating CI from NCI after stroke and TIA at six months was at 24/25, and it is important for routine clinical practice.

Background: Stroke upsets the quality of life of the patients and their families. The participation of the family in caring for these patients is inevitable. Empowerment programs enhance patients' motivation and knowledge and improve their quality of life and self-care. This research aimed to determine the effect of a program based on the family-centered empowerment model on the quality of life of patients with stroke. Methods: This clinical trial study was performed on 100 patients with stroke in Shahid Beheshti Hospital in Kashan City, Iran. The subjects were first recruited purposefully and then were randomly divided into two groups. The experimental group received a family-centered empowerment training in 8 sessions, while the control group received no intervention. The patients in the two groups completed the stroke-specific quality of life scale before the intervention, immediately after the intervention, and one and two months after the last training session. The obtained data were analyzed using the Chi-square test, repeated measures ANOVA, and t-test in SPSS V. 19. Results: Comparison of the experimental and control groups showed that the total score of quality of life significantly changed immediately after the intervention, and one and two months later (P<0.05). Repeated measures ANOVA results reported a significant interaction between time and group in the total quality of life score and some of its dimensions (energy, family role, social role, language, mobility, self-care, upper extremity function, and work/productivity) (P≤0.05). Conclusion: This model of empowerment can improve the quality of life of patients with stroke. This model is recommended to use in the educational program of these patients.
