Objectives: The aim of the study was to assess the parental perceptions of silver diamine fluoride (SDF) dentinal staining using a pre-tested questionnaire and SDF treatment photographs. Materials and Methods: In this cross-sectional study, 300 parents of pre-school children with previous dental cavity experience were randomly selected to obtain data. Univariate analysis of the association of variables and the acceptability of SDF based on the treatment was conducted using the Mann–Whitney U test, P < 0.05 was considered statistically significant. SDF treated photographs of anterior and posterior teeth were displayed and their understanding of the ease of application and dentinal staining was obtained using a pretested, self-structured, and closed-ended questionnaire by face-to-face interview method. Results: After interviewing 163 mothers and 137 fathers, 62.4% of participants acknowledged SDF staining on the posterior teeth to be satisfactory, but only 28.7% of parents made this equivalent judgment about anterior teeth (P < 0.005). In the absence of their child having behavioral barriers to conventional restorations, 60.3% of parents reported that they were likely to settle on SDF to treat their child’s posterior teeth, but only 21% of participants were likely to settle on SDF to treat their child’s anterior teeth. The level of acknowledgment of the parents increased with the children’s behavioral barriers. Conclusion: Although staining on anterior teeth was undesirable, most parents opted for this cavity management over advanced behavioral techniques. Therefore, the proper understanding of parental perceptions is important while choosing SDF for treating dental caries.

Background: Silver Diamine Fluoride (SDF) has emerged as a promising alternative to the traditional caries management concept of ‘drill and fill’. It offers a conservative, inexpensive, painless and simple way to stop caries progression. Aim: To assess the knowledge and use of Silver Diamine Fluoride among dental practitioners in Lahore. Methodology: A cross-sectional study was conducted after getting institutional ethical approval. A pre-validated questionnaire was used to collect data from 186 dental practitioners. Data was analyzed using SPSS V 25. Results: Among the participants, 101(54.3) were females and 83(44.6%) were males. 30.6% and 47.8% of the participants strongly agreed and agreed to the use of SDF to arrest cavitated lesions. Regarding the barriers in SDF usage, permanent black discoloration was chosen by 70.4%, concern over patient satisfaction by 55.7% and the fact that SDF not restoring tooth anatomy if it is not followed by a restoration by 40.3% of the participants. 72.6% of the participants reported that they have never used SDF, 20.2% sometimes and 7.2% often use SDF. 55.9% of the participants expected an increase infuture usage of SDF. Conclusion: Dentists have a fair knowledge about the use of SDF and its application in clinical practice. However, there is disparity in its use among the dental practitioners due to the black discoloration and concern over parental acceptance. Introduction: Evidence-based noninvasive caries therapies for initial caries lesions recently became available in the United States. Fundamental differences between noninvasive therapies and the traditional surgical dental approach warrant study of the financial scalability. Methods: The financial costs and benefits to fee-for-service clinicians and payors were compared across eleven scenarios simulating the treatment of 1,000 initial lesions over a three-year period. The scenarios included varying combinations of noninvasive therapies (silver diamine fluoride (SDF), Curodont Repair Fluoride Plus, and glass ionomer sealants), no treatment, and various rates of one to three surface restorations to an estimated current practice model. We used a decision tree microsimulation model for deterministic and probabilistic sensitivity analyses. We derived assumptions from an initial lesion and noninvasive therapy-focused cohort study with operations data from 16 sites accepting Medicaid in Alabama as a case study and clinical data from all 92 sites. Results: In comparison to the current practice model, scenarios that produce mutually beneficial results for payors in terms of cost savings and clinics in terms of net profit and profit margin include: Curodont, SDF on non-cosmetic surfaces, and a mix of three noninvasive therapies. When considering the limited resources of chair and clinician time, the same scenarios as well as SDF with restorations emerged with vastly higher profit margins. Conclusion: Scenarios that include noninvasive therapies and minimize restorations achieve the balance of improving outcomes for all parties. Reaping these benefits requires payors to appropriately reimburse and clinics to adopt the noninvasive therapy procedures.
