Most Adult Intussusceptions are Caused by Tumors: A Single-Centre Analysis

Abstract: Background: Adult intussusception is less common than paediatric intussusception. The aim of this study was to explore the clinical presentation, aetiology, diagnosis and treatment of adult intussusception. Methods: Adults (>18 years) with intussusception treated by surgical or conservative measures were included from January 2005 to January 2018, and the manifestation, types, diagnosis and treatment of adult intussusception in our centre were reviewed. Results: A total of 150 patients with adult intussuscepti… Show more

“…Even though small intestine metastases are a rare and life-threatening clinical entity, they occur more frequently than primary small intestine tumors[ 6 ]. A recent study found that tumors are the primary causes of intussusception in adults[ 7 ]. Intussusception is the invagination of a segment of the gastrointestinal tract into an immediately adjacent area.…”

“…Colonic intussusceptions in adults are induced by malignancy as the lead point in 60% of cases, while only 30% of small bowel intussusceptions are caused by malignant tumours. [7][8][9] Benign causes of small bowel intussusception are predominantly diverticular disease (Meckel's) and polyps of various origins. Lipomas are rarely encountered and comprise only 1% of all gastrointestinal tumours.…”

“…Intussusception is the most common cause of bowel obstruction in the pediatric population [1][2][3][4][5]. Intussusception is rare in adults with an estimated incidence of 2 cases/1,000,000 population/year, accounting for only 5% of all intussusceptions, 1% of all bowel obstructions, and 0.003-0.02% of all hospital admissions [2][3][4][5][6][7][8][9]. Amongst the Adult Intussusceptions (AI), 38-44% occur in the colon and 52-55% in the small intestine [4,6].…”

“…Due to this variable presentation, AI is difficult to diagnose, despite the evolution of imaging procedures and often remains unrecognized or misdiagnosed prior to surgery [2,5,10]. Abdominal Ultrasound (US) and Computed Tomography (CT) scan have high sensitivity for the diagnosis of AI but limited value in discriminating whether a lead point is malignant, benign, or idiopathic [3,10].…”
