A case of persistent bacteraemia by Ralstonia mannitolilytica and Ralstonia pickettii in an intensive care unit

Abstract: The Ralstonia spp . genus is a group of non-fermentative, Gram-negative bacteria often resistant to many antibiotics, which are emerging as opportunistic pathogens frequently associated with infections in hospital settings. We present herein a case of combined R. pickettii and R. mannitolilytica persisting and relapsing bacteraemia, possibly caused by a septic arterial thrombosis secondary to the rupture of an internal carotid artery aneurysm… Show more

“…Ralstonia spp. rarely cause infection in healthy individuals but can be a severe pathogen especially in immunosuppressed patients[38]. Therefore, the presence of R. mannitolilytica in stable COPD patients may be an important contributing factor in predisposing patients to recurrent infection and exacerbations.…”

“…Tests for nitrate reduction (negative in R.mannitolilytica ) and acidification of D-arabitol and mannitol (both negative in R.pickettii ) can differentiate the 2 species [ 11 ]. Matrix assisted laser desorption and ionisation time of flight methods such as MALDI TOF MS (Biomerieux, France) has also been found to yield a more accurate species level identification of Ralstonia species [ 4 ]. 16S rRNA gene sequencing has proved very useful in the definitive identification of R.mannitolilytica , and is now considered the reference method for identification to species level [ 4 , 12 ].…”

We used the Vitek 2 instrument for identification and this may explain why Ralstonia picketii was identified from the water specimen as opposed to Ralstonia mannitolilytica from clinical specimens.…”

“…COPD patients present altered airway microbiome, with Ralstonia mannitolilytica identi ed in a signi cantly higher rate comparing with healthy population [4]. Despite its commonly overlooked role as a commensal, R. mannitolilytica becomes an emerging global opportunistic human pathogen and a causative agent of various infections and diseases, including bacteremia, meningitis, sepsis, peritonitis, osteomyelitis, hemoperitoneum and urinary tract infection [5][6][7][8][9][10]. Furthermore, R. mannitolilytica is also identi ed in respiratory illnesses such as cystic brosis and COPD, presumably as colonizer [11,12].…”
