Shewanella algae Bacteremia in an End-stage Renal Disease Patient: A Case Report and Review of the Literature

Abstract: A 71-year-old man was admitted because of nausea and abdominal pain. He was receiving an erythropoiesis-stimulating agent for anemia and dysregulated iron metabolism due to stage G5 chronic kidney disease. He had a history of raw fish intake and was diagnosed with infectious enterocolitis, which worsened and led to septic shock. Shewanella putrefaciens grew in the blood culture, but Shewanella algae was identified in a 16S rRNA gene sequence analysis. We herein report a case of S. algae bacteremia believed to … Show more

“…are responsible for a wide range of clinical conditions such as skin and soft tissue infections, bacteremia, gastroenteritis, cerebellar abscesses, ear and eye infections, infective endocarditis and pericarditis. Direct contact to seawater or consumption of contaminated seafood are well-known risk factors for infections (1,3,12,13). Pneumonia, cerebellar abscess, meningitis and wound infections were the reported cases from our country (14)(15)(16).…”

“…Risk factors predisposing to Shewanella infection include the aforementioned underlying diseases such as hepatobiliary disease, malignancy and end stage renal disease (ESRD). A recent study conducted by Takata et al 3 described that dysregulated iron metabolism due to the aforementioned comorbidities increases the risk of Shewanella infection 3. Our patient’s underlying hepatobiliary disease and prior history that revealed iron overload may explain the causation of his Shewanella infection.…”

“…Risk factors and comorbidities associated with Shewanella infections have been described as chronic leg ulcers, peripheral vascular disease, diabetes, chronic liver disease and kidney disease. Exposure to aquatic environments2 and raw fish ingestion3 have also been defined as significant risk factors leading to infection.…”

“…In recent years there has been an increase in the reports on Shewanella spp. isolated from clinical samples establishing it as an emerging opportunistic pathogen 1317 . This bacterium is also known for its potential application in bioremediation, its versatile metabolism, and its genetic plasticity 12 .…”
