Lowered Circannual Urinary Melatonin Concentrations in Episodic Cluster Headache

Abstract: The circannual secretion of melatonin in 14 Swedish and 15 Italian patients suffering from episodic cluster headache was compared with 14 Swedish and 15 Italian healthy controls matched for sex and age. Overnight samples of urine were collected once a month from 8 to 14 months and kept at -20 degrees C until analysed with RIA. The melatonin concentrations in nocturnal urine were permanently low in cluster headache and there was no consistent change of the melatonin concentration in relation to cluster periods … Show more

“…Melatonin stimulates Thelper 1 (Th1) cell cytokines, among them interferon-γ (INF-γ) and interleukin-2 (IL-2) [28][29][30][31]. IL-2 is reported to be at lower levels in CH sufferers [32], fitting with the observation that plasma melatonin levels are also too low during a cluster period [33,34]. At night, a disruption of the Th1/melatonin signaling, caused by lower-than-normal melatonin levels, could signal the beginning of a CH attack.…”

“…В ряде исследований у пациентов, страда-ющих кластерной головной болью, во время ремиссии определяется более низкий уровень мелатонина по срав-нению со здоровыми людьми [29,30]. Одной из гипотез возникновения кластерной головной боли является гипо-таламическая дисфункция [31], а применение экзогенно-го мелатонина, предположительно, может оказать поло-жительный эффект.…”

“…2 Neuroendocrinologic alterations point to participation of biologic clock generators in the pathophysiology. The secretion of melatonin, which is mainly produced by the pineal gland and which plays a major role in the synchronization of the sleep/wake cycle, has been shown to be altered in CH, 3 and oral administration has been suggested for the treatment of CH. Regarding the opioidergic system, the circadian rhythm of beta-endorphin serum levels is lacking in patients with CH, 4 and opioids can alter the secretion of melatonin by the mammalian pineal gland.…”

“…Voxels were considered to be significant if they reached a statistical threshold of p Ͻ 0.05 after correction for multiple comparisons on cluster or whole-brain level. Because we expected changes in the pineal gland and hypothalamus, small volume correction (SVC; p Ͻ 0.05) was applied on these structures (spherical volumes of interest centered around the peak coordinate, radius 10 mm, volume 4189 mm 3 ). We performed an additional statistical parametric mapping analysis (unpaired t test) of the episodic patients vs controls with identical thresholds as described for the whole patient population.…”
