Patients with varicose veins of the lower extremities with osteoarthritis of the knee often experience improvement in knee joint symptoms after endovascular treatment. We considered that it was important to decide the operation indication of lower extremity varices, to know the correlation between the two diseases in the treatment of varicose veins. To investigate the postoperative improvement of knee symptoms related to varicose veins with knee osteoarthritis, we conducted a questionnaire survey for a total of 12 months, from December 2014 to May 2015 and from October 2018 to March 2019. The participants were 35 patients (7 men and 28 women) with varicose veins complicated with knee osteoarthritis. We classified knee osteoarthritis according to a grading scale and compared the improvement of knee symptoms after endovenous thermal ablation. The higher the knee grade, the lower the degree of improvement. However, the improvement was observed in all knee osteoarthritis grades, and as a whole, 25 patients (71.4%) have experienced improvement of subjective symptoms. For patients with knee osteoarthritis, we strongly recommend surgical treatment of the varicose veins regardless of the progression of knee grade. Objectives: This study aims to evaluate whether there is a relationship between venous insufficiency (VI) and knee osteoarthritis (OA). Patients and methods: Between February 2012 and May 2013, a total of 206 knees of 103 participants (14 males, 89 females; mean age 48.6±8.6 years; range, 30 to 69 years) were enrolled. The study group included 59 patients who were diagnosed with knee OA and the control group included 44 healthy volunteers without any complaint in their knee joints. Demographic and clinical characteristics of all participants were recorded. Venous system of the lower extremities was evaluated by Doppler ultrasonography. All knees were evaluated using conventional radiography based on the Kellgren-Lawrence (K&L) grading system and ultrasonography. Pain severity was evaluated using Likert pain scale and functioning was evaluated using Western Ontorio and McMaster Universities Osteoarthritis Index (WOMAC). Results: Venous insufficiency was detected in 40.6% of the OA group and 15.9% of the control group (p=0.007). There was no statistically significant difference in the cartilage thicknesses and K&L grading regarding the presence of VI (p>0.05); however, the percentage of the radiographic medial tibial sclerosis was higher in patients with VI (60%) in the OA group (p>0.05). The WOMAC total scores were similar in both groups, while the WOMAC pain scores were higher in the patients with deep VI (p>0.05). Conclusion: Increased radiographic medial tibial sclerosis and higher WOMAC pain scores in patients with venous involvement in OA may lead to the hypothesis that venous system pathologies can affect the intraosseous microenvironment of the bone, resulting in pain and early subchondral bone involvement, consequently presenting as subchondral sclerosis.

Objective: to determine risk factors (RFs) for venous thromboembolic events (VTE) in patients with rheumatoid arthritis (RA).Subjects and methods: The investigation enrolled 374 patients (311 women and 63 men) with a reliable diagnosis of RA who met the 2010 ACR/EULAR classification criteria. The patients' mean age was 53.7±13.6 years; the disease duration was 12.1±10.7 years. All the patients were treated at the V.A. Nasonova Research Institute of Rheumatology Clinic during the period from 2014 to 2017. A standard clinical examination of the peripheral joints was performed. RA activity was measured using DAS28. A survey was made using a questionnaire including questions on traditional RFs for VTE and RFs that might be caused by RA and its therapy. Results and discussion. VTE were recorded in 45 (12%) out of the 374 patients. Group 1 included 45 patients with VTE; Group 2 consisted of 329 patients without VTE. Multidimensional analysis showed an increased risk of developing VTE in RA under the influence of the following factors: high inflammatory activity; lower extremity varicose veins; hypercholesterolemia; and hypertension. Their weighted coefficients were 1.1, 2.5, 1.0, and 0.9, respectively. According to the obtained model (p<0.001), the risk of VTE in RA can be predicted by the following formula: Z = 1.1 • high RA activity (Yes = 1/No = 0) + 2.5 • lower extremity varicose veins (Yes = 1/No = 0) + 1.0 • hypercholesterolemia (Yes = 1/No = 0) + 0.9 • hypertension (Yes = 1 / No = 0).Conclusion: The increased risk for VTE in RA patients determines the need for its timely assessment, by taking into account the known risk factors as both standard ones and those caused by the disease itself and its therapy. This risk assessment is necessary for the timely adequate treatment and prevention of thrombotic events in RA.
