Lower doses of rFVIIa therapy are safe and effective for surgical interventions in patients with severe FXI deficiency and inhibitors

Abstract: Severe FXI deficiency is a rare injury-related bleeding disorder. In patients with FXI inhibitors, surgeries may be treated using recombinant activated factor VII; however, treatment safety is a major concern and the best dosing regimen as well as mode of administration is still to be defined. We describe four patients with severe factor XI deficiency and inhibitors to FXI, undergoing eight (four major) surgical procedures treated with continuous infusion of rFVIIa. Following acute MI that evolved after surger… Show more

“…733 Low-dose rFVIIa (33 to 47 mg kg À1 ) also appears to be well tolerated and effective for surgery in patients with severe factor XI deficiency and inhibitors. 734 Coadministration of tranexamic acid has also proved effective, 735,736 although it may increase thrombotic risks. Elsewhere, effective haemostasis was reported in 100% of factor XI deficient patients receiving prophylactic rFVIIa before dental procedures, and minor or major surgery.…”

“…Reported interventions include immunosuppression (Teruya & Styler, 2000), plasma exchange (Teruya & Styler, 2000). rFVIIa 15-30 lg/kg in combination with tranexamic acid prevented bleeding in major surgery in cases with FXI inhibitors, without venous thrombosis (Kenet et al, 2009;Livnat et al, 2009).…”
