Low prevalence of hypothyroidism among black and Mulatto people in a population‐based study of Brazilian women

Abstract: Objective African-Americans have been shown to have low prevalence of hypothyroidism. Brazil has a high ethnic admixture allowing further exploration into whether environmental factors can explain the ethnic differences. Design A survey, representative of the population of Rio de Janeiro, a large metropolitan city in Brazil. Factors studied included race, parity, income, schooling, and smoking. Population The survey was carried out in Rio de Janeiro whereby households (1500) were selected using three-stage pro… Show more

“…In the study by Rosa da Silva and cols., (8) a lower prevalence (4%) for positive anti-thyroperoxidase antibodies (TPO-Ab) was found among 117 HIV-infected patients compared to the expected for the general population (14), but it is in line with a previous published study in the same population (15). This result is surprisingly considering the high prevalence (12.8%) of subclinical hypothyroidism found in this sample, and suggests an environmental protective factor in HIV-infected patients to thyroid autoimmunity.…”

“…In recent studies, similar prevalence of positivity, 12.4% to 14% (35,43,106) Although the presence of TPOAb usually precedes the development of thyroid dysfunction, recent studies suggest that a hypoechoic ultrasound pattern may precede a biochemical TPOAb abnormality (108,109) (A,B) The paradoxical absence of TPOAb in some patients with unequivocal TSH abnormalities likely reflects the suboptimal clinical and methodological sensitivity and/or specificity of current TPOAb tests (25,109) (B,B).…”
