Low-Pressure Cardiac Tamponade

Abstract: Low-pressure cardiac tamponade was identified in 20% of patients with catheterization-based criteria of tamponade. Clinical recognition may be difficult because of the absence of typical physical findings of tamponade in most patients. Although some patients are critically ill, most show a stable clinical condition. However, these patients obtain a clear benefit from pericardiocentesis.

“…Twenty-one patients fulfilled 1 criterion for low-pressure cardiac tamponade, ie, intrapericardial pressure Ͻ7 mm Hg at baseline during tamponade. 17 Hemodynamic parameters during pericardiocentesis are summarized in Table 1 (Table 2). As shown in Table 2, compared with patients whose cardiac output did not increase after volume expansion, patients in whom volume expansion resulted in an increase in cardiac index Ͼ10% had a lower arterial pressure and lower cardiac index before volume expansion.…”

“…The intra-pericardial pressure exceeded the right atrial pressure ‘earlier’ or at ‘lower pressure’ than seen in cases without volume depletion [9]. In a retrospective analysis, low-pressure cardiac tamponade was identified in 20% of patients with catheterization-based criteria of tamponade [10]. …”

“…Quando uma quantidade significativa de líquido se acumula e ultrapassa a capacidade de distensão do tecido fibroelástico pericárdico, ocorre progressiva compressão de todas as câmaras cardíacas decorrente do aumento da pressão intrapericárdica, redução do volume de enchimento cardíaco e maior interdependência ventricular 175 .…”
