ObjectivesEmerging evidence indicates that tendon disease is an active process with inflammation that is critical to disease onset and progression. However, the key cytokines responsible for driving and sustaining inflammation have not been identified.MethodsWe performed a systematic review of the literature using MEDLINE (U.S. National Library of Medicine, Bethesda, Maryland) in March 2017. Studies reporting the expression of interleukins (ILs), tumour necrosis factor alpha (TNF-α) and interferon gamma in diseased human tendon tissues, and animal models of tendon injury or exercise in comparison with healthy control tissues were included.ResultsIL-1β, IL-6, IL-10, and TNF-α are the cytokines that have been most frequently investigated. In clinical samples of tendinopathy and tendon tears, the expression of TNF-α tended not to change but IL-6 increased in tears. Healthy human tendons showed increased IL-6 expression after exercise; however, IL-10 remained unchanged. Animal tendon injury models showed that IL-1β, IL-6, and TNF-α tend to increase from the early phase of tendon healing. In animal exercise studies, IL-1β expression showed a tendency to increase at the early stage after exercise, but IL-10 expression remained unchanged with exercise.ConclusionsThis review highlights the roles of IL-1β, IL-6, IL-10, and TNF-α in the development of tendon disease, during tendon healing, and in response to exercise. However, there is evidence accumulating that suggests that other cytokines are also contributing to tendon inflammatory processes. Further work with hypothesis-free methods is warranted in order to identify the key cytokines, with subsequent mechanistic and interaction studies to elucidate their roles in tendon disease development.Cite this article: W. “…The effect of infrared radiation on nerves has been shown in the literature in relation to the photobiological mechanisms of infrared LED, which has an anti‐inflammatory action, such as the modulation of cyclooxygenases and lipoxygenases and the reduction of inflammatory mediators such as prostaglandins, prostacyclins, bradykinin, histamine, substance P, interleukin 1 beta, and tumor necrosis factor alpha, decreasing the activation of the nociceptors . In addition, many studies have shown satisfactory results in relation to the effects of infrared, near‐infrared, or both wavelengths on different pathological conditions such as burn wounds, postsurgery pain, Achilles tendinopathy, hip arthroplasty, knee osteoarthritis, and oral mucositis, where there is an inflammatory process. It is very important to understand whether the chromophores activated by different wavelengths are correlated with the individual responses or eventual cumulative response of both wavelengths in the same treatment.…”

Low‐Intensity Photobiomodulation Decreases Neuropathic Pain in Paw Ischemia‐Reperfusion and Spared Nervus Ischiadicus Injury Experimental Models

Background There is a wide range of animal models available today for studying chronic pain associated with a variety of etiologies and an extensive list of clinical manifestations of peripheral neuropathies. Photobiomodulation is a new tool for the treatment of pain in a convenient, noninvasive way. Objective The aim of this work is to elucidate the effects of infrared light‐emitting diodes (LEDs) on behavioral responses to nociceptive stimuli in chronic pain models. Methods Forty‐eight Swiss male mice weighing 25 to 35 g were used. Two chronic pain models, ischemia‐reperfusion (IR) and spared spinal nerve injury, were performed and then treated with infrared LED irradiation (390 mW, 890 nm, 17.3 mW/cm2, 20.8 J/cm2, for 20 minutes). The behavioral tests used were a mechanical hypersensitivity test von Frey test) and a cold allodynia test (acetone test). Results The results showed that, in the IR model, the infrared LED had a significant effect on mechanical stimulation and cold allodynia on every day of treatment. In the spared nerve injury model, an analgesic effect was observed on every treatment day (when started on the 3rd and 7th days after the surgery). In both models, the effect was abolished when the treatment was interrupted. Conclusions These findings suggest that photobiomodulation therapy may be a useful adjunct treatment for chronic pain.

Abstract: Our results demonstrate that cryotherapy in combination with LLLT can produce an anti-inflammatory "add-on" effect. The order of therapy administration seems essential, as superior histology and biomechanical results were found in the cryotherapy first/LLLT group.
