The aim of this study was to investigate the effect of laser phototherapy (LPT) in the prevention and/or treatment of oral mucositis induced by 5-fluorouracil (5-FU; Eurofarma, São Paulo, Brazil) in hamsters. Ninety-six hamsters were divided into four groups (n = 24): Control (no treatment); Preventive [LPT from day (D) D−5 to D+5]; Therapeutic (LPT from D+5 to D+15); and Combined (preventive plus therapeutic LPT from D−5 to D+15). The animals received an intraperitoneal injection of 5-FU on Days 0 and 2. The pouch mucosa was scratched on Days 3 and 4. The irradiation parameters were: indium-gallium-aluminum-phosphide (InGaAlP) diode laser (MM Optics, São Carlos, Brazil) (660 nm), beam area of 0.036 cm², 40 mW, 1.11 W/cm², 6.6 J/cm², power density applied daily of 39.6 J/cm², in punctual mode (six points and six seconds per point) and contact mode, one application per day. The animals were sacrificed on Days 0, 5, 10 and 15 (n = 6) and weighed, and the pouch mucosa was removed for histopathological analysis. Clinical and corresponding histological scores were compared using ANOVA and Tukey's test (p ≤ 0.05). Similar weight losses ranging from 5% to 10% occurred in all groups. The therapeutic group had significantly lower clinical and histological scores than the other groups at Day 10. This study showed that positive effects on oral mucositis management were obtained only when LPT was applied in the therapeutic protocol (from D+5 to D+15 after chemotherapy).

ResumoIntrodução: o câncer é considerado uma doença genética, na qual os genes que sofreram mutações por motivos físicos ou químicos, modificam a função biológica natural. Os métodos mais eficazes para a destruição de células tumorais ainda são a quimioterapia e a radioterapia, porém, seus efeitos colaterais, na maioria das vezes, interferem na qualidade de vida dos pacientes. Dentre as diversas complicações envolvidas no tratamento antineoplásico, destaca-se a mucosite oral (MO). Objetivo: a presente revisão de literatura objetiva descrever a etiopatogênese da MO e destaca as principais complicações provenientes deste tipo de lesão. Metodologia: artigos científicos que abrangeram pesquisas clínicas, relatos de caso clínico e revisões de literatura foram pesquisados nas bases de dados BIREME e PubMed, entre os anos de 2000 a 2015, através dos descritores específicos. Resultados: por se tratar de um processo inflamatório que acomete a mucosa oral e estar associada a quadros de desconforto e dor, é importante tentar proporcionar ao paciente afetado por esta condição clínica alívio da sintomatologia dolorosa. Há na literatura o registro do emprego de algumas medidas profiláticas e terapêuticas para a mucosite associada à oncoterapia, tais como uso de agentes físicos e químicos. O laser, por exemplo, é uma forma de radiação não ionizante e com alta concentração, não invasiva e bem tolerada pelo organismo. Conclusão: é de grande importância a realização de estudos sobre o tema para que se tenha dados e parâmetros de análise suficientes para melhorar a qualidade de vida dos pacientes em tratamento contra o câncer. INTRODUÇÃOA mucosite oral (MO) é uma resposta inflamatória aguda da mucosa que se desenvolve pela utilização de drogas antineoplásicas ou radiação ionizante em região de cabeça e pescoço, que são usados no tratamento do câncer. 1, 2,3Grande parcela dos pacientes portadores de câncer é submetida à radioterapia, cirurgia e/ou quimioterapia. A radioterapia é, geralmente, o tratamento de escolha para os casos que envolvem cabeça e pescoço, onde o campo de irradiação afeta a mucosa oral e as glândulas salivares 4,5 . Por isso, entre os pacientes com câncer de cabeça e pescoço submetidos à radioterapia, 90-97% deles apresentam algum grau de MO. 2,5 A MO pode ameaçar a eficácia do plano de tratamento, e desta forma, provocar a interrupção da radioterapia e a diminuição das doses dos agentes antineoplásicos

Sa prise en charge est avant tout symptomatique : prescription d'un traitement antalgique, topique et systémique. Depuis quelques années, l'utilisation de différents lasers s'est révélée particulièrement intéressante pour le traitement préventif et curatif des mucites radio et/ou chimio-induites. L'objectif de cet article est de faire le point sur les mécanismes d'action du laser de basse énergie et ses indications dans le traitement de lésions muqueuses spécifiques. Nous rapportons ici l'exemple d'un cas de mucite de grade 3 radio-induite chez un patient en cours de radiothérapie postopératoire pour un carcinome épidermoïde du rebord maxillaire. La prise en charge classique (soins locaux et antalgiques) a été mise en place, en association avec des séances de laser Nd :YAP (1,34 μm). Le retour à une toxicité muqueuse ≤ 2 et une cicatrisation de la plupart des zones ulcérées ont été observés dès la première semaine d'application. Parallèlement, un effet antalgique rapide était constaté avec une diminution de la douleur qui est passée de 90 à 30 sur l'EVA.Key words: oral mucositis / laser / pain / head and neck cancers Abstract -Lasers in the treatment of oral mucositis. Report of a case and review of littérature. The mucositis is a common complication of radiotherapy and/or chemotherapy in the treatment of head and neck cancers. Its management is symptomatic with topical and systemic analgesics. In recent years, the use of various lasers has proved particularly interesting for the preventive and curative treatment of mucositis induced by radiotherapy and chemotherapy. The aim of this article is to review the mechanisms of action of low energy laser and its indications in the treatment of mucosal lesion. We report here the case of mucositis grade 3 radiation-induced in a patient during postoperative radiotherapy for squamous cell carcinoma of the maxillary. Classical treatment (local care and analgesics) was set up in conjunction with sessions of Nd:YAP (1.34 mm). The return to ≤ 2 mucosal toxicity and scarring of the ulcerated areas were most observed in the first week of application. Meanwhile a rapid analgesic effect was observed with a decrease in pain that went from 90-30 on the VAS.La radiothérapie, exclusive ou postopératoire, occupe une place fondamentale dans le traitement des cancers des voies aéro-digestives supérieures.
