Introduction: In patients with hypoparathyroidism, conventional therapy maintains parathyroid hormone (PTH)-dependent mineral metabolism homeostasis but is unable to prevent emergence of comorbidities and low quality of life. Objectives: To evaluate long-term progression of patients with hypoparathyroidism receiving conventional therapy and their quality of life compared with patients with primary hypothyroidism and healthy controls. Design and Setting: Retrospective cohort study for quality-of-life analysis and transversal cut on clinical profile. Patients with hypoparathyroidism from four public referral centers in endocrinology and bone metabolism in the metropolitan region of Rio de Janeiro. Material and Methods: Quality of life by SF-36 protocol, and clinical profile by medical record analysis. Results: 243 individuals with hypoparathyroidism (n=113), hypothyroidism (n=65), and healthy controls (n=65) included. Median time since diagnosis and duration of conventional therapy was 8 years (IQR 4–17 years). Data on type of conventional therapy (median, minimum–maximum daily dose, percentage of patients with hypoparathyroidism using each medication): calcium supplementation (2000 mg/day, 200–6000 mg/day, 95%), cholecalciferol (2000 IU/day, 200–40000 IU/day, 44%), calcitriol (0.5 μg/day, 0.25–2 μg/day, 77%), thiazides (25 mg/day, 12.5–100 mg/day, 44%). Conclusions: Conventional therapy is associated with homeostasis of serum mineral levels, but not with improved quality of life. Compared to patients with hypothyroidism, those with additional hypoparathyroidism had lower scores in six SF-36 domains. Conventional therapy successfully maintained normal calcium levels with often high doses of calcium, vitamin D, and thiazides but could not prevent low quality of life scores and comorbidities.

Hypoparathyroidism is an endocrine disorder characterized by low or absent production of parathyroid hormone (PTH). The most common cause of hypoparathyroidism is injury to the parathyroid glands or unintentional removal during thyroid surgery. The author aims to explore further the early detection and diagnostic approaches for hypoparathyroidism. The search was conducted through the Pubmed database using the keywords "Detection AND Diagnose AND Hypoparathyroidism." A total of 183 results were obtained. However, after applying inclusion and exclusion criteria, 7 publications were used in this literature review. The diagnosis of hypoparathyroidism is established through the assessment of clinical symptoms, physical examination, and laboratory tests.

Objective. Hypoparathyroidism (HPt) is a rare endocrine disorder often of postsurgical origin, resulting in hypocalcaemia. Several complications have been described including impairment of quality of life (QoL). Our study aims to evaluate the effect of hypoparathyroidism on the QoL of patients diagnosed with HPt. Methods. A cross-sectional study was conducted in the Department of Endocrinology, Diabetology, Metabolic Diseases and Nutrition of the Hassan II University Hospital of Fez. We included in our study all patients followed for chronic HPt. Well-being was assessed using the WHO 5 index, and QoL was assessed by the SF-36 questionnaire in its validated Arabic version. Data were entered into Excel and analysed using SPSS 26. Multiple linear regression was utilized to ascertain the variables linked to the QoL in individuals diagnosed with HPt. Results. A total of 143 patients with HPt were included in the study, 86.7% of whom were female. The mean age of the patients was 44.6 ± 17.3 years. 89.9% were of postsurgical etiology. The assessment of well-being by the WHO 5 index showed a low score (<50), meaning poor well-being in 44.8%. Regarding the QoL, the assessment showed low scores in the areas of general health (41.7), limitations due to physical condition (40.5), vitality (41.4), and limitations due to psychological condition (42.6). The multiple linear regression model revealed a noteworthy association between low SF-36 score and advanced age (β = −5.91; p<0.001), surgical etiology (β = 8.71; p<0.001), low education level (β = −10.1; p<0.001), and poor compliance with medication (β = −11.3; p<0.001). However, the relationship between impaired QoL and achievement of normocalcemia was nonsignificant (p=0.69). Conclusions. Our work objective is that patients with HPt have a reduced and multifactorial QoL. Despite normocalcemia, it is hypothesized that parathyroid hormone directly influences QoL. These results could serve as a basis for future research.
