Background/Aim:Helicobacter pylori (H pylori) plays an important role in the pathogenesis of chronic gastritis, peptic ulcer disease, and gastric neoplasms. Therefore, it is necessary to select an effective regimen for H pylori eradication. The aim of this study was to compare the efficacy of two quadruple-therapy regimens—one with azithromycin and the other with metronidazole—for H pylori eradication in patients with dyspepsia.Materials and Methods:In this double-blind randomized clinical trial conducted in Rasoule-Akram Hospital in 2006, we included 60 patients (aged 15–70 years) who had dyspepsia and H pylori infection as diagnosed by upper gastrointestinal endoscopy and rapid urease test. Patients were randomly assigned to receive a quadruple-therapy regimen for 2 weeks: 1) the MAO-B group (n= 30) received metronidazole 500 mg b.i.d, amoxicillin 1g b.i.d, omeprazole 20 mg b.i.d, and bismuth 240 mg b.i.d and 2) the AAO-B group (n = 30) received azithromycin 500 mg once daily for 1 week and amoxicillin 1g b.i.d, omeprazole 20 mg b.i.d, and bismuth 240 mg b.i.d for 2 weeks). H pylori eradication was assessed by the rapid urease test (RUT) 2 months after the cessation of treatment.Results:H pylori was eradicated in 68% and 69% of patients in the MAO-B and AAO-B groups, respectively. There was no significant difference in H pylori eradication rates between the two groups (P = 0.939).Conclusion:No significant difference exists between the two quadruple-therapy regimens that were tested.

Preparation of Nano-Medicine to Eliminate Helicobacter pylori Infection

Helicobacter pylori (H. pylori) is considered a challenging type of bacteria that is difficult to treat with the currently used antibiotics, such as amoxicillin, erythromycin, and metronidazole, which have proven ineffective against these bacteria. In this study, modern technology was used to treat these bacteria by converting the aforementioned antibiotics to their nano state using the lyophilization method and diagnosing them using scanning electron microscopy. A mixture of the three nano-antibiotics was prepared in the form of a nano-medicine, which was used to treat H. pylori bacteria in cultures and determine the effectiveness of nano-antibiotics and nano-medicine on these bacteria. The findings showed that nano-medicine was highly effective in inhibiting these bacteria at the lowest concentration (OD=0.042) and the highest concentration (OD=0.038), compared to the three micro-antibiotics individually. The OD values of amoxicillin, azithromycin, and metronidazole were 0.523, 0.521, and 0.453, respectively. The OD values of the three nano-antibiotics, including nano-amoxicillin, nano-azithromycin, and nano-metronidazole, were 0.386, 0.258, and 0.167, respectively. It was observed that the percentage of inhibition in each of the nano-antibiotics was higher than the inhibition in micro-antibiotics and that the nano-medicine had much higher inhibition than each of the three micro- and nano-antibiotics alike. The safety of using nano-antibiotics in the prepared medicine was confirmed using electrochemical technology and cyclic voltammetry to identify the electrochemical properties through oxidation and reduction in blood media. Based on the findings, only reduction peaks appeared, and there were no oxidation peaks in the prepared kit or for each of the three nano-antibiotics. It was found that they were all non-oxidants and could be used safely as good antioxidants in treatments. However, the same three micro treatments showed blood oxidation due to the appearance of oxidation peaks in all of them. The study proved that all H. pylori isolates are resistant to usable antibiotics. All of the antibiotics in the nano-medicine had an anti-bacterial effect, and the effect of the new form of antibiotic was proportional to the concentration of the antibiot

Background: The Brazilian consensus recommends a short-term treatment course with clarithromycin, amoxicillin and proton-pump inhibitor for the eradication of Helicobacter pylori (H. pylori). This treatment course has good efficacy, but cannot be afforded by a large part of the population. Azithromycin, amoxicillin and omeprazole are subsidized, for several aims, by the Brazilian federal government. Therefore, a short-term treatment course that uses these drugs is a low-cost one, but its efficacy regarding the bacterium eradication is yet to be demonstrated. The study's purpose was to verify the efficacy of H. pylori eradication in infected patients who presented peptic ulcer disease, using the association of azithromycin, amoxicillin and omeprazole.
