Loop-and-Let-Go: Treatment of a Large Colonic Lipoma After Unsuccessful Unroofing

Abstract: Gastrointestinal lipomas are benign subepithelial tumors arising from adipose tissue. Most colonic lipomas are clinically silent and do not require intervention. However, if the lipomas are large or symptomatic, removal can be considered. Of the endoscopic techniques available, endoscopic ligation, also known as “loop-and-let-go,” provides a safe and easy alternative to other endoscopic therapies and does not require any specialized equipment or advanced training. We describe a case of a patient found to have … Show more

“…Conversely, recent studies suggest that unroofing, due to its perceived low-risk nature, should be considered the primary treatment option, particularly for lipomas exceeding 20 millimeters in size or in cases involving patients with concurrent medical conditions [ 4 ]. Combining endoscopic treatment methods is also endorsed when initial interventions are insufficient [ 20 ]. Despite advancements in endoscopic treatment, colectomies remain the preferred option for addressing these benign lesions in many cases.…”

“…Two step methods via a subsequent EMR, or the loop and go technique have been described. We found this technique to be a safe, one-step approach for the treatment of these lesions, which also allowed to obtain a histological sample to confirm that the lesion was benign and no other treatment was needed 1 2 3 4 5 .…”
