Background.Primary intrahepatic adenosquamous carcinoma (iASC) is a very rare subtype of cholangiocarcinoma (CCA), with a worse prognosis than adenocarcinoma. Hepatectomy is still the first choice for iASC, and liver transplantation (LT) for iASC has not been previously reported.Case presentationThe young male patient with an unresectable primary iASC underwent a LT using a marginal fatty liver graft. The overall survival of this patient was 16 months, with a recurrence-free survival of 8 months. The patient had a good quality of life with normal liver function until 3 months before death.Conclusions.This is the first case report of LT as treatment of unresectable iASC using a marginal graft. Although the recipient may benefit from LT, LT for iASC should proceed with caution owing to its more aggressive features.

A case of acute cholecystitis with abnormally high CA19-9

Background CA19-9 is strongly expressed in malignant tumors of the digestive system and is widely used as a marker for gastrointestinal cancer. In this report, we describe a case of acute cholecystitis in which CA19-9 was markedly elevated. Case presentation A 53-year-old man was admitted to our hospital with a diagnosis of acute cholecystitis after being referred to our hospital with a chief complaint of fever and right hypochondrial pain. CA19-9 was abnormally high at 17,539.1 U/ml. Although the possibility of malignancy was considered, there was no obvious malignant lesion on imaging; the patient was diagnosed with cholecystitis, and laparoscopic cholecystectomy was performed the day after admission. The surgical specimen showed no malignant findings either grossly or in the final pathological examination. There were no complications in the patient’s postoperative course, and he was discharged from the hospital on the third postoperative day. CA19-9 level quickly returned to within normal range after surgery. Conclusions In acute cholecystitis, CA19-9 levels exceeding 10,000 U/ml are very rare. We report a case of acute cholecystitis without malignant findings despite a high CA19-9 level.

Cholangiocarcinoma is a rare type of cancer disorder. The case report is of a 55-years-old male patient who came to the output patient department (OPD) with complaints of abdominal pain and weight loss for 15 days and later was diagnosed with Klatskin tumor after mandatory investigations. After the approval of the tumor board committee, he is under chemotherapy, to which he is responding positively. Klatskin tumor is a type of cholangiocarcinoma occurring at the convergence of the right and left lobes of hepatic bile ducts, forming the common bile duct. The source of cholangiocarcinoma is idiopathic, and most cholangiocarcinoma is treated with the help of either surgical resection or chemotherapy. Surgical resection is performed in initial cases, but most patients present with the advanced stage of Klatskin tumor. CA-19-9 is a tumor marker that indicates the presence of a Klatskin tumor.
