The authors combined this with minimal requirements for exposure characterization allowing for analysis of population-based studies. Focusing on spirometric criteria for airflow obstruction, this review of occupational COPD includes both population-wide and industry-specific exposures.Methods We used PubMed and Embase to identify relevant original epidemiological peer-reviewed articles, supplemented with citations identified from references in key review articles. This yielded 4528 citations. Articles were excluded for lack of lung function measurement, insufficient occupational exposure classification, lack of either external or internal referents, non-accounting of age or smoking effect, or major analytic inadequacies preventing interpretation of findings. A structured data extraction sheet was used for the remaining 147 articles. Final inclusion was based on a positive qualitative Scottish Intercollegiate Guidelines Network (SIGN) score (≥2+) for study quality, yielding 25 population-wide and 34 industry/occupation-specific studies, 15 on inorganic and 19 on organic dust exposure, respectively. ResultsThere was a consistent and predominantly significant association between occupational exposures and COPD in 22 of 25 population-based studies, 12 of 15 studies with an inorganic/mineral dust exposure, and 17 of 19 studies on organic exposure, even though the studies varied in design, populations, and the use of measures of exposure and outcome. A nearly uniform pattern of a dose-response relationship between various exposures and COPD was found, adding to the evidence that occupational exposures from vapors, gas, dust, and fumes are risk factors for COPD. ConclusionThere is strong and consistent evidence to support a causal association between multiple categories of occupational exposure and COPD, both within and across industry groups.Key terms: airflow obstruction; COPD; dust; fume; gas; lung function measurement; occupational exposure; vapor. The aim of the present study was to investigate the occurrence of new respiratory symptoms in relation to wood dust exposure in a 6-yr follow-up study.A total of 1,377 woodworkers (1,137 males; 240 females) and 297 reference workers (137 males; 160 females) participated. Data on respiratory symptoms, employment and smoking habits were collected. Wood dust exposure was assessed from baseline dust measurements, and cumulative wood dust exposure was assessed by study-specific job exposure matrices and exposure time.The geometric mean (geometric SD) dust level decreased during the study period from 0.94 (2.1) to 0.60 (1.6) mg?m -3 . Adjusted analysis revealed positive associations for cumulative incidence proportion of chronic bronchitis and daily coughing for female woodworkers versus female reference workers. The cumulative incidence proportion of daily coughing and chronic bronchitis were found to be associated with baseline wood dust exposure in a dose-dependent manner. The odds ratio (95% confidence interval) for daily coughing (with reference to the lowest exposure quartile) was 1.6 (0.6-4.3), 3.2 (0.9-6.8) and 3.8 (1.5-9.7), respectively, in the second and third lowest and the highest quartile. The figures for chronic bronchitis were, accordingly, 2.3 (0.4-14.5), 3.0 (0.5-18.7) and 6.0 (1.2-28.8).In conclusion, female woodworkers in this low exposure cohort showed an increased incidence of coughing and bronchitis, whereas no relations to wood dust exposure were seen for male woodworkers.

Occupational exposure to wood dust is associated with an increased risk of respiratory diseases including acute or chronic impairment of lung function, cancers of the nasal cavity, lung, digestive system, and hematopoietic system, asthma, and allergic and nonallergic dermatitis. Most recent studies on occupational wood dust exposures have been focused on respiratory symptoms, lung function, and sinonasal cancer. The specific causative agents, and so on, wood dust alone (natural products), wood dust with additives used in the processing or manufacturing of wood products, and/or physical and chemical properties of wood dust, associated with adverse effects are still poorly understood. This chapter discusses the peer‐reviewed scientific literature published on occupational exposure to wood dust and the associated risks of developing adverse health effects. In future studies, attention is called for robust exposure assessment that includes control of confounding variables, duration of exposure, and sufficient follow‐up time for onset of effects. This information is needed to understand the causation of health effects caused by wood dust exposure and the demographic differences in disease rates as well as to develop strategies for interventions to reduce risks to wood industry workers.
