Longitudinal Circulating Tumor DNA Analysis in Blood and Saliva for Prediction of Response to Osimertinib and Disease Progression in EGFR-Mutant Lung Adenocarcinoma

Abstract: Background: We assessed whether serial ctDNA monitoring of plasma and saliva predicts response and resistance to osimertinib in EGFR-mutant lung adenocarcinoma. Three ctDNA technologies—blood-based droplet-digital PCR (ddPCR), next-generation sequencing (NGS), and saliva-based EFIRM liquid biopsy (eLB)—were employed to investigate their complementary roles. Methods: Plasma and saliva samples were collected from patients enrolled in a prospective clinical trial of osimertinib and local ablative therapy upon pro… Show more

“…Tumor DNA can reach the saliva either through direct shedding from a mucosal tumor or filtering from plasma within salivary glands [ 31 ]. This notion that saliva is a filtered version of blood plasma has been supported by observations that liquid biopsy genotyping of lung cancer is concordant between plasma and salivary DNA [ 32 ].…”

“…This approach is not only suitable for primary tumor tissue but is routinely used in the transbronchial needle aspiration of lymph node [ 87 ], plasma [ 88 ], and pleural effusions [ 89 ] and could also be used from bronchoalveolar lavage fluid [ 90 ] and is also suitable for cerebrospinal fluid [ 91 ]. Besides, there are promising data for the screening of residual disease of EGFR mutant NSCLC in saliva [ 92 ] and urine [ 93 ].…”

“…Lung cancer is the most commonly diagnosed malignancy worldwide and lung adenocarcinoma (LUAD) represents the most common histological type of lung cancer (69)(70)(71)(72)(73). A late diagnosis of LUAD contributes to high metastasis and mortality rates, emphasizing the urgency for better identification of sensitive biomarkers during lung cancer progression (69,(74)(75)(76).…”
