Longest duration of retention of video capsule: A case report and literature review

Abstract: Video capsule endoscopy (VCE) is a safe innovative tool for investigating obscure gastrointestinal bleeding, Crohn's disease and other small bowel pathologies. The capsule is usually excreted with faeces within 24-48 h. Retention of capsule rarely occurs, and it usually depends on the indication of VCE. The longest reported case of capsule retention in the literature is 2.5 years. Surgical approach is considered effective to retrieve the retained capsule. We present a case of asymptomatic retention of capsule … Show more

“…[5] Most retained capsules are asymptomatic with the longest duration of asymptomatic retention reported being 4.5 years. [6] In a retrospective study of 104 retained capsules only 15% had partial small bowel obstruction. [7] In another retrospective study of 1000 VCE with 1.4% retention rate, all retentions were asymptomatic.…”

“…Bhattarai et al reported a case of 4 years and 5 months asymptomatic capsule retention in a 49-year-old man who had subtotal colectomy with ulcerative colitis, which was resolved by colonoscopy procedure. [13] If endoscopy is not successful, a small number of patients may require surgical intervention, whereas asymptomatic patients may initially be put under observation. [14] However, simple follow-up in asymptomatic patients does not always produce good results.…”

“…Most retained capsules are asymptomatic with the longest duration of asymptomatic retention reported being 4.5 years. 14 For retained capsule conservative treatment should be considered when there is no need for immediate surgery by treating the underlying cause, that is, use of anti-inflammatory agents or colonoscopy preparation fluids, which may lead to spontaneous passage.…”
