Long-term Survival Rates in Young Patients With Thyroid Carcinoma

Abstract: Thyroid carcinoma in patients younger than 18 years has a good prognosis even in the presence of neck or distant metastasis. Total thyroidectomy, associated with adjuvant radioactive iodine therapy and thyroidal suppression or not, is effective in patients with a well-differentiated thyroid carcinoma.

“…In multistep carcinogenesis, poor prognosis in the elderly is explained to be due to genomic damages accumulated in cancer cells after many years of proliferation, enabling cancer cells to acquire more aggressive characteristics. However, this hypothesis is not convincing since thyroid cancers in the young cause invasion or metastasis in a similar manner to those in the elderly [41]. Thyroid carcinomas in the young seem to be self-limiting, even though invasion or metastasis is observed frequently.…”

“…However, unlike PTC, FTC is prone to early hematogenous metastases, which occurs even in the absence of cervical node involvement (383,384). Despite that, conventional FTC has an excellent prognosis when diagnosed during childhood, and long-term survival is the norm (8,385,386).…”

“…Thyroid cancers in young patients grow rapidly and frequently accompany distant metastasis. Even in such cases, thyroid cancers in the young are associated with an excellent prognosis and, after appropriate therapies, no patient dies from cancer [27,28]. On the contrary, thyroid carcinomas in elderly patients with distant metastasis show a poorer prognosis, compared with those without it [29].…”
