Long-term survival and late graft loss in pediatric liver transplant recipients—a 15-year single-center experience

Abstract: Increasing numbers of children undergo successful liver transplantation. Limited data exist on long-term survival and late graft loss. Survival and graft loss were studied in 376 primary liver graft recipients who survived more than 3 months after transplantation (80.5% of all primary graft recipients). Patient records were reviewed retrospectively for causes of graft loss. Risk factors were identified by analyzing graft, recipient, and posttransplant variables using multivariate Cox regression. One-, 5-, and … Show more

“…Those patients who experience more than one episode of acute cellular rejection were found to have an almost twofold increase in LGL, a finding that reaffirms recent studies that demonstrate an association between chronic rejection and recurrent acute cellular rejections (8,11). Similar to other studies, steroid-resistant rejection was also found to be associated with LGL (8).…”

“…Similar to other studies, steroid-resistant rejection was also found to be associated with LGL (8). Although tacrolimus has been shown to dramatically decrease the rate of chronic rejection in pediatric liver-transplant recipients (9-12), analysis of the SPLIT data did not show any difference in LGL in those children receiving cyclosporine versus tacrolimus (data not shown).…”

“…Current long-term survival rates are over 80%. 1,2 However, few data are available regarding the long-term histological outcome of transplanted allografts in children. Studies in adults have revealed histological abnormalities in allografts but have focused largely on patients with liver dysfunction or late graft loss.…”

“…Eine belgische Studie berichtete bei 376 Kindern, welche zwischen 1984 und 2000 transplantiert wurden, über ein Ein-, 5-und 10-Jahres-Überleben von 95,7, 91,4 und 90,4% [14]. In einer Serie aus Hamburg lag das Patientenüberleben nach 6 Monaten bei 100% [15].…”
