Long-Term Surgical Outcome of Posterior Choroidal Melanoma Treated by Endoresection

Abstract: These data suggest that transvitreal endoresection of posterior uveal melanoma is an acceptable management option to conserve the globe and functional vision in selected patients. Distant metastasis is an infrequent event in this modality of treatment.

“…The surgical technique used in our patients is similar to the techniques reported in other studies 9 10 12. We apply certain surgical measures to minimise the risk of local recurrence and metastasis, as was described in 2001,10 and attribute our relatively low rates of local recurrence and metastasis to these measures.…”

“…6 In the present series, the mean follow-up time was 8.5 years, the longest reported in the endoresection literature. The study by Karkhaneh et al 18 This study is nonrandomized, which makes comparison with other therapeutic modalities less reliable. However, we can affirm that the local recurrence risk at 5 years in our series is higher than with 125I radiotherapy, proton beam radiation, and 106 Ru plaque brachytherapy, but lower than with transscleral resection and the COMS medium tumor study results.…”

“…9,23 The rates of other commonly described complications, such as cataracts, ocular hypertension, and epiretinal proliferation were similar to our results. 9,11,17,18 Additional procedures are often needed after endoresection, such as silicone oil removal, or cataract surgery. Patients should be informed of this possibility before giving their consent for endoresection.…”

“…Eye-preserving treatment strategies (surgery, radiotherapy, and laser treatment) have now become popular. [1][2][3][4][5][6][7][8][9] Local tumor control rates currently reach more than 90%. 2,[4][5][6]8,9 Small and medium-sized UMs are usually treated with transpupillary thermotherapy or plaque brachytherapy.…”
