Long-Term Results of Treatment of 283 Patients with Lung and Bone Metastases from Differentiated Thyroid Carcinoma

Abstract: We assessed the results of treatment in 283 patients with lung or bone metastases from differentiated thyroid carcinoma who were followed for up to 40 yr (median, 44 months) after the discovery of the metastases. The survival rates from the time of discovery of the metastases were 53% at 5 yr, 38% at 10 yr, and 30% at 15 yr; 156 patients died. Multivariate analysis revealed that only 4 variables had an independent prognostic significance for survival. They were extensive metastases, older age at discovery of t… Show more

“…The proponents for more radical surgery (total or subtotal) thyroidectomy are based on theoretical disease control benefits: (1) papillary carcinoma and its subtypes are frequently multifocal; (2) there is a reduced incidence of local and distant recurrence; (3) reduced risk of anaplastic transformation in the thyroid remnant, and (4) it facilitates of post-thyroidectomy monitoring by 131 I scan and serum Tg levels [9]. In addition, residual thyroid tissue after lobectomy may interfere with the use of radioactive iodine in the treatment of microscopic regional and distant disease.…”

“…The 10-year survival rate following the detection of bone metastasis is less than 20%. 113,117 Thyroid carcinoma bone metastases typically have an osteolytic phenotype; however, it is believed that osteoblastic metastasis of thyroid carcinomas may have a better prognosis. 1 …”

“…However, because 4-7% of patients present initially with distant metastasis (Schlumberger et al 1986, Shaha et al 1997, and surgery is the only therapeutic option for thyroid cancer, advocating the routine use of PET to detect distant metastasis is inappropriate. In addition, because PET scanners have limited spatial resolution, nodules in the lung smaller than 5 mm may be missed (Jeong et al 2006a).…”
