Long-Term Results of Carpal Tunnel Decompression

Abstract: The long-term prognosis of 60 patients operated on for carpal tunnel syndrome has been assessed in a prospective study with a median follow-up period of 5.5 years (range 2-11 years). Analysis of motor, sensory, trophic, and electrodiagnostic findings and assessment of pain were performed pre- and post-operatively using a standardized grading system. The results were generally favourable with a variable degree of improvement in 86% of cases. Statistical evaluation using multiple Dunn-Rankin tests revealed pain … Show more

“…6 Several papers have also been published showing specifically that carpal tunnel decompression in patients with diabetic neuropathy will give a significant improvement to those patients. [36][37][38] When managing patients with neuropathic pain because of peripheral neuropathy, it is very important to work closely with pain-management experts who can optimize their medical management using the various drugs that can modulate peripheral sensitization, descending inhibitory pathways, and central sensitization. One the most common treatments today for peripheral neuropathic pain is gabapentin (Neurontin), which was originally shown to be of great benefit in diabetic-induced neuropathy.…”

“…Therapeutic effects are well-known: steroid treatment provides temporary benefit [6,7] while surgical decompression is considered the only approach that offers the possibility of a definitive cure [1]. The post-operative evolution has been described [2,4,8], Nevertheless, few data are available on the natural course of untreated CTS [9,10]. We prospectively evaluated the natural clinical and neurophysiological course of 80 hands afflicted by CTS.…”
