“…Options for revascularization are coronary artery bypass or percutaneous intervention, each presenting its own pitfalls. Frequently, the diffuse nature of the vasculopathy and vessel thickening precludes bypass grafting, while percutaneous coronary intervention has been shown to have higher rates of in-stent restenosis, target vessel revascularization, myocardial infarction and death as compared to non-transplant recipients 3 .…”

“…The optimal treatment for allograft vasculopathy in OHT patients is unknown, but PCI appears to be a therapeutic option. One of the main concerns of PCI in OHT patients is re-stenosis, as the allografted coronary vessel is in a proinflammatory state, which influences vessel remodeling (13)(14)(15)(16). Multivariate analysis revealed that in allograft vasculopathy, the use of stents decreased the risk of restenosis (odds ratio 0.34, p<0.05) compared with cutting balloon or other PCI methods (10,17).…”

“…Although in a study comparing long-term results of stenting and angioplasty in allograft vasculopathy stenting has a lower restenosis rate at 6 and 12 months follow-up, 5 year restenosis rates were similar. 7 We performed PTCA without stenting. Stent implantation was not done because of the ectasia in this vessel segment, the diameter of the vessel was above 4.0 mm.…”
