Background: Nose reconstruction following resection of nasal carcinomas is controversial. The objective of this study is to investigate the effect of surgical reconstruction versus prosthetic rehabilitation on patient quality of life (QOL). Design: This was a monocentric prospective study of patients diagnosed with nasal carcinoma from 2003 to 2013. QOL was evaluated using two organ-specific questionnaires (Rhinoplasty Outcome Evaluation [ROE] and the Functional Rhinoplasty Outcome Inventory-17 [FROI-17]) and a generic questionnaire, the Short-Form 36 Health Survey (SF-36). Material and Methods: Sixty-four patients were included. Patients completed the ROE, FROI-17, and SF-36 questionnaires after nasal reconstruction. Questionnaires were completed by 62.8% of the 51 alive patients. Results: Recurrence-free survival (RFS) was 89.9%, disease-specific survival was 94.5%, and overall survival was 75.5% after five years according to the Kaplan-Meier method. Considering initial tumor stage, early stage patients had a significantly higher selfconfidence score in FROI-17 subgroup analysis. In contrast, advanced stage patients showed a significantly higher score for social functioning in SF-36. Prosthetically fitted patients scored highly on the ROE questionnaire showing a high degree of aesthetic satisfaction. Surgically reconstructed patients showed a high degree of self-confidence on the FROI-17 questionnaire. However, the organ-specific ROE and FROI-17 scores were not significantly different between patients who received surgical reconstruction and prosthetic rehabilitation after oncological resection. When comparing the rehabilitation method as a function of tumor stage, there was significantly better score for "physical functioning" in early stage surgically reconstructed patients in the SF-36, but no significant differences in organ-specific QOL. Conclusion: Surgical reconstruction and prosthetic rehabilitation after nasal cancer resection have the same effect on organ-and non-organ-specific QOL.

ZusAmmenfAssungIn kaum einem anderen Bereich ist das Ergebnis so offensichtlich, wie bei der plastisch-ästhetischen Nasenchirurgie. Umsomehr spielen sowohl die Qualität des Eingriffs und im Bereich der Ästhetik die Qualität des Ergebnisses eine herausragende Rolle. Hierbei sind die Erwartungen und der Anspruch des Operateurs nicht immer deckungsgleich mit der des Patienten. Des Weiteren haben Kostenträger, im Falle von Kombinationsoperationen, einen eigenen Fokus zum Thema Qualität, der häufig mit Wirtschaftlichkeit verwechselt wird. Objektive Kriterien spielen für den Arzt eine wesentliche Rolle, wobei für die Patienten sehr häufig weiche Kriterien von wesentlicher Bedeutung sind. Dies ist umso diffiziler, da Schönheit im Auge des Betrachters liegt und mit funktionellen Kriterien, wie Atmung und Riechen vereinbar sein müssen. In der Vielzahl von Qualitätskriterien fällt es dem Laien schwer die richtige Arztwahl zu treffen, insbesondere angesichts unzähliger Siegel, Zertifkate und des Einflusses des Internets bzw. von Social Media. Aber auch der Chirurg muss aus einer Vielzahl von Kongressen, Kursen und Symposien herausfinden, welche dieser Veranstaltungen eine bestimmte Mindestqualität garantiert. Trotz einer enormen Veränderung und Verfeinerung der rhinochirurgischen OP-Techniken im Rahmen der letzten Jahrzehnte, wird die Ergebnisqualität seitens der Patienten konstant vor die Prozedurqualität gestellt. Dieses Referat versucht eine Standortbestimmung möglichst objektivierbarer Qualitätsindikatoren in Kombination mit unterschiedlichen Anforderungen und Standpunkten zum Thema Qualität -aus Sicht des Arztes, des Patienten und des Kostenträgers -zu beleuchten und diese im Spiegel sich verändernder OP-Techniken der plastisch-ästhetischen Nasenchirurgie zu betrachten. Both the quality of the procedure and the quality of the result play an outstanding role in the arena of aesthetics. Expectations and interests of the surgeon are not always congruent with those of the patient. Furthermore, payers, in the case of combination surgery, have their own focus on quality, which is often confused with economy. Objective criteria play an essential role for the physician, with soft criteria being very important for patients. This is much more difficult because beauty is in the eye of the beholder and must be compatible with functional conditions such as breathing and smelling. In the variety of quality standards, it is difficult to make the right choice of surgeon for the non-professional, especially in the face of countless seals, certificates and the influence of the internet and social media. But even the surgeon has to ascertain a minimum level of quality from a large number of congresses, courses, and symposia. ZusammenfassungUm mit der Unvollkommenheit ärztlichen Handelns umzugehen, gibt es neben gesellschaftlichen Sanktionen schon seit der Antike ethische Verhaltensvorgaben für Ärzte, die unser heutiges Verständnis von einem guten Arzt geformt haben. Mit Industrialisierung und Standardisierung wurde vor 100 Jahren in den USA der Grundstein für Qualitätsmanagement in der Medizin gelegt. In den 1950ern entstand in Japan ein umfassendes Qualitätskonzept, dass großen Einfluss auf die Medizin gewinnen sollte. Alle heutigen Zertifizierungs- und Akkreditierungsverfahren gehen aus diesen Wurzeln hervor.Seit 15 Jahren knüpft der Gesetzgeber in Deutschland ein immer feineres Netz an Regularien, um durch externe Qualitätssicherung Mindeststandards im Gesundheitswesen zu gewährleisten. Ärztliche Institutionen der Selbstverwaltung verbessern darüber hinaus durch eigene Initiativen die Behandlungsqualität.Qualitätsmanagement hat dauerhaft Einzug gehalten in Krankenhäuser und Praxen, zu erkennen an Risikomanagement und Patientenorientierung. Daneben gibt es eine Vielzahl an freiwilligen ärztlichen Initiativen zur Qualitätssicherung, z. B. die Leitlinien der ärztlichen Fachgesellschaften.Per Umfrage wurde die Implementation eines Qualitätsmanagementsystems in deutschen HNO-Kliniken und -Praxen evaluiert und mit dem Bundesdurchschnitt verglichen. Es fand sich überwiegend eine umfassendere Umsetzung als in den Vergleichsgruppen.Die aktuellen Herausforderungen an Kliniken und Praxen sind erheblich und haben Auswirkungen auf das Qualitätsniveau. Unterfinanzierung, Personalmangel und gesellschaftliche Veränderungen sind hier genauso relevant wie Auswirkungen des wissenschaftlichen Fortschritts.Um Qualitätsmanagement zukünftig umfassend zu implementieren, sind kluge politische Weichenstellungen und Ressourcen entscheidend – das Konzept an sich hat sich längst durchgesetzt.
