Long-term prognostic significance of response in multiple myeloma after stem cell transplantation

Abstract: For establishing the true effect of different response categories in patients with multiple myeloma (MM) treated with autologous stem cell transplantation, we evaluated, after a median follow-up of 153 months, 344 patients with MM who received a transplant between 1989 and 1998. Overall survival (OS) at 12 years was 35% in complete response (CR) patients, 22% in near complete response (nCR), 16% in very good partial response (VGPR), and 16% in partial response (PR) groups. Significant differences in OS and pro… Show more

“…However, the significant improvement in overall survival has mainly been observed in younger MM patients [11][12][13][14][15][16][17], whereas advances for elderly MM patients have been less significant [11,14,16,18].…”

“…Unfortunately, disease relapse or progression of disease post ASCT remain inevitable for most patients irrespective of the timing of the ASCT. 1,2 However, the clinical features at the time of relapse and subsequent clinical outcomes are quite heterogeneous amongst these patients. This information is particularly important given that it may provide new benchmarks when comparing different therapies in the post-ASCT relapse setting in clinical trials.…”

“…Although the same has been shown in the setting of autologous transplantation, [32][33][34] the meaning of CR in the allogeneic transplantation setting is distinct since the ultimate goal is longterm cure. This is underlined by data showing the prognostic Allogeneic transplantation in multiple myeloma S Gerull et al significance of reaching molecular remission, 35,36 and suggests that molecular monitoring should be considered after allogeneic transplantation.…”
