Long-Term Outcomes After Total Pancreatectomy and Islet Cell Autotransplantation

Abstract: This represents one of the largest series examining long-term outcomes after TPIAT. This operation produces durable pain relief and improvement in QOL parameters. Insulin independence rates decline over time, but most patients maintain stable glycemic control.

“…Several centers, including our own, have even been able to demonstrate the longterm durability of this procedure. 6,11,12 One question that has yet to be addressed is the role that total or completion pancreatectomy and islet cell autotransplantation (CPIAT) has in managing patients with refractory chronic pancreatitis after failed previous pancreatic resection and/or decompression. At our institution, CPIAT is used as salvage therapy for patients with refractory disease after traditional operative intervention.…”

“…Affirmation of the safety of the procedure along with short and long-term durability of pain relief with narcotic independence, improved quality of life, and insulin independence have been shown in multiple studies [13][14][15][16] with narcotic-independence rate at 73% at 5-year follow-up in one study [17]. A systematic review conducted by Bramis et al, on TPIAT for CP in a total of 296 patients from 72 studies reported a significant reduction in pain as well as an insulin-independence rate ranging from 46% at 5 years to 10% at 8 years [18].…”

“…This was based on the insightful study of Nealon and Walser [17]. series [18] show that only 38% of children are insulin independent at 1-year follow up, and the percentage decreases with time (27% at more than 5 years). On the other hand, Chinnakotla et al [19] showed good results with statistically improved pain relief in 90% of their patients after TP-IAT (P < 0.001).…”
