Long-term outcome of transcanalicular microdrill dacryoplasty: a minimally invasive alternative for dacryocystorhinostomy

Abstract: Background/AimDacryocystorhinostomy (DCR) remains the gold standard therapy for nasolacrimal duct obstruction (NLDO), but is invasive and does not maintain the physiology of the lacrimal pathway. With transcanlicular microdrill dacryoplasty (MDP), there is a minimally invasive alternative surgical approach. This study aimed to present this modern lacrimal duct surgery technique and to evaluate its long-term success rate in a large study population.MethodsThe medical records of 1010 patients with acquired NLDO … Show more

“…While long-term data on DCR show a success rate of around 90%, the only publication with a similar follow-up period after transcanalicular dacryoendoscopy showed a success rate of just under 60%, although the initial success rate was 84% 11 . When comparing success rates of different studies on nasolacrimal duct surgery, the definition of success and the careful and correct indication for the performance of microendoscopic procedures need to be reviewed critically 11 . In our study only a complete resolution of epiphora (corresponding to a Munk score of grade 0) was defined as success.…”

“…Data on the recurrence of symptoms in these patients were also assessed by a telephone survey in the period from 2017 to 2019. In this study, only the complete resolution of symptoms ("no epiphora") without subsequent dacryocystitis, inflammation of the lacrimal apparatus or any additional surgical intervention during the follow-up period was defined as success, corresponding to a Munk score of grade 0 11 , 12 .…”

“…Although dacryocystorhinostomy using a nasal endoscope for patients with chronic dacryocystitis was first reported in 1989 and the first clinical study of endoscopic dacryocystorhinostomy was published in 1994, the success rates of early nasal endoscopic operations were not high. Improvements in endoscopic equipment and techniques [ 3 ] and further knowledge of lacrimal duct anatomy have increased the usefulness of nasal endoscope technology in ophthalmology [ 4 ], with endoscopic dacryocystorhinostomy becoming routine in lacrimal duct operations [ 5 ].…”

“…Im Rahmen einer TWE wird dabei nach Rekanalisierung zur Vermeidung postoperativer Verklebungen üblicherweise eine Intubation mit einem Silikonschlauch für die Dauer von ca. 3 Monaten vorgenommen [6,7]. Bei der externen DCR wird die Notwendigkeit einer Intubation der Tränenwege dagegen weiterhin diskutiert [8].…”
