Long-Term Outcome of Open or Endovascular Repair of Abdominal Aortic Aneurysm

Abstract: Conclusion:Long-term mortality is high after endovascular repair of abdominal aortic aneurysm (AAA). In the setting of endovascular AAA repair, the Lee Index may be useful for stratifying short-term and long-term mortality in high-risk patients.Summary: Perioperative myocardial infarction remains a vexing problem. In very high-risk vascular surgical patients, perioperative myocardial ischemic injury may be as high as 18% to 35% (J Vasc Surg 2006;43:533-538). Although a large majority of patients survive a peri… Show more

“…12 These findings are consistent with the more recent report on a six-year follow-up of patients undergoing either EVAR or OPEN repair of AAA. 13 At the same time, interestingly, the number of EVAR procedures has increased, particularly in octogenarians. 14 Although the etiology of delirium is multifactorial, the reasons behind the higher prevalence of delirium in the OPEN group were likely related to the extent of surgical trauma, consequent systemic inflammatory response, and oxidative stress associated with more invasive procedures.…”

“…These results were also substantiated by the EUROSTAR Register (European Collaborators on Stent/Graft Techniques for Aortic Aneurysm Repair) [22]. After a 6-year follow-up period, long-term results of an additional prospective study (n = 351) show continued equivalent results between endovascular and open surgical repair [23]. In contrast to elective aneurysm repair, only non-randomized data regarding the treatment of ruptured AAAs is available.…”

“…Although operative mortality is lower, the rate of re-intervention is significantly higher than with open surgery due to continued risk for developing late endoleaks (2,27).…”

“…Descending thoracic aneurysms (DTA) and abdominal aortic aneurysms (AAA) are now routinely repaired using endovascular techniques (1,2). Operative mortality for endovascular abdominal aneurysm repair (EVAR) is less than 2% (3).…”
