Objective In Japan, percutaneous endoscopic gastrostomy (PEG) has been used mainly in patients with stroke and dementia, who undertake oral ingestion voluntarily. We have used PEG for patients with various diseases in Saga Medical School Hospital, including postoperative recovery, malignant disease, and neurodegenerative diseases. This study evaluated prognostic factors in these patients regarding long-term survival. Methods We analyzed retrospectively all patients who received PEG at our hospital. During the period of 1998-2007, 84 patients (32 females, 52 males; mean age, 60.3 years, range 20-89 years) were followed for more than 1 year. We analyzed sex, age, total lymphocyte count, serum albumin level, presence of malignant diseases, cerebrovascular diseases, neurodegenerative disorders, poor general condition after surgical procedures, dementia before PEG, pneumonia before PEG, and complications of PEG placement. Results As for diseases, 23 patients had malignant diseases, 27 had cerebrovascular diseases, 19 had neurodegenerative disorders, 16 were in poor general condition after surgery for nonmalignant diseases, and 12 had dementia. Multivariate analysis indicated that risk factors for 1-year survival were low serum albumin level (!2.9 g/dL), low lymphocyte count, and complications of malignant diseases. Low serum albumin level, low lymphocyte count, and malignant diseases were risk factors, and only the albumin level was a risk factor in those without malignant diseases. Conclusion Low serum albumin level was a risk factor for 1-year survival with PEG, which suggests that nutrient management before and during PEG placement should be monitored carefully.

Objective: In this study, we aimed to evaluate the results and experiences of the patients who received percutaneous endoscopic gastrostomy (PEG). Methods:A total of 113 patients who admitted to the Dicle University Medical Faculty , Department of Gastroenterology between January 2012 and December 2014 and in whom received PEG was performed. The patients were assessed in terms of indications, complications and results.Results: Among these patients, 70 (61.9%) were male and 40 (38.1%) were female. Though 8 (7%) patients had head, neck and esophageal cancer; 105 (93%) patients had primer or seconder neurological disorders. After the PEG, any serious complication was seen in patients. Wound infections were encountered in five patients (4.4%) and the rate of minor complications was found to be 9.7%. The risk of complications was higher in patients over sixty years and men (p values of 0.049 and 0.022). Conclusion:Percutaneous endoscopic gastrostomy, a simple and safe method of enteral nutrition with a low complication rate, should be the first choice when extended period enteral nutrition is required. There is increased risk of complications in elderly males.

ÖzetAmaç: Oral yoldan beslenemeyen ancak gastrointestinal sistem fonksiyonları normal olan yaşlı hastalarda perkütan endoskopik gastrostomi (PEG) işle-minin güvenilirliğini değerlendirmek, hastaların vücut kitle indeksi (VKİ) ile iş-lem süresi ve komplikasyon oranları arasındaki ilişkiyi belirlemektir. Gereç ve Yöntem: 2011 Şubat ve 2014 Aralık ayları arasında çekme yöntemi ile yü-zeyel anestezi altında PEG yerleştirilen, 65 yaşın üstündeki 97 hastanın klinik verileri ve komplikasyon oranları retrospektif olarak incelendi. Tüm hastalar yaş, cins, PEG endikasyonları, komplikasyonlar, işlem süreleri, antibiyotik profilaksi uygulanması, vücut kitle indeksi (VKİ) ve takip süreleri açısından değerlendirildi. Bulgular: Çalışmamızda 51'i (%52,6) kadın, 46'sı(%47,4) erkek toplam 97 hasta mevcuttu. Yaş ortalaması 72 ± 4 idi. PEG endikasyonu olarak 41(%42) olguda serebrovasküler hastalıklar, 19(%19,5) olguda obstrük-siyon yapan malign hastalıklar, 16(%16,5) olguda demans ve 21(%22) olguda diğer hastalıklar saptandı. Vücut kitle indeksi ortalama 25±3(17-32)kg/m² olarak hesaplandı. Ortalama işlem süresi 8,6±1,6 dakika idi. VKİ 25'in üzerin-de olanlarda işlem sürelerinde istatistiksel anlamlı artış saptandı (p<0,05). Toplam 10(%10,2) hastada minör komplikasyon saptandı. 8(%8) olguda antibiyotik tedavisi gerektirecek yara çevresi enfeksiyonu, 2(%2) olguda kateter kenarından hafif sızdırma görüldü. VKİ 25'in üstünde olanlarda komplikasyon oranının istatistiksel anlamlı yüksek olduğu saptandı (p<0,05). Hastaların takip süresi 224(68-660) gün olup işleme ait mortalite saptanmamıştır. Tartışma: PEG, çeşitli nedenlerle oral yoldan beslenemeyen olgularda uzun süre-li ve en uygun beslenme desteği sağlayan güvenli bir yöntemdir. VKİ yüksek hastalarda, işlem sırasında komplikasyon riski açısından daha özenli uygulama gerekebilmektedir. Material and Method: Between 2011 February and 2014 December, we retrospectively analyzed the clinical data and complication rates of 97 geriatric patients to whom PEG was applied by the pull method under superficial anesthesia. Results: 51(53%) of our patients were female and 46(47%) were male with a mean age of 72 ± 4. Abstract: Mortality of patients who underwent percutaneous endoscopic gastrostomy appears to be high. In particular, the long-term prognosis was poor for patients with hypoalbuminemia, and this procedure is probably not preferred for patients with recurrent aspiration pneumonia.
