“…Azathioprine has been found eff ective in maintaining remission in controlled and uncontrolled drug withdrawal studies ( 231,232 ) and in a meta-analysis of seven placebo-controlled maintenance trials ( 233 ). Retrospective studies have shown the value of 6-MP and azathioprine in maintaining long-term remission ( 200,234 ), and it is generally well tolerated during long-term use ( 197,200,201,234 ). As with induction of remission in UC, there have been no studies comparing 6-MP with azathioprine.…”

“…However, experience with the thiopurines over the last four decades indicates that there is not an increased risk of the development of solid tumors (as discussed above) ( 201 ) or overall mortality ( 200,238 ). Conversely, a recent meta-analysis of six cohort studies calculated a fourfold increased risk of lymphoma among IBD patients treated with thiopurines, but it remains unclear whether this risk was due to the medications themselves or due to the underlying disease ( 176,180,239 ).…”

“…e risk of lymphoma related to purine analogs has been widely debated (15,17,216,217) . Aside from a potential risk of lymphoma (216) , neoplasia has not been observed with the use of purine analogs for in ammatory bowel disease (218,219) and it is accepted that the documented bene ts are most likely to o set a small increased risk (220) . Lymphomas that have occurred during thiopurine therapy have increasingly been recognized to be related to Epstein -Barr infections (216,217,221) .…”

“…The fact that the highest mutation frequencies in Crohn's disease patients occurred in the two who had received azathioprine would be predicted from treatment with a known mutagen, and raises concerns that azathioprine therapy may add to the carcinogenic risk in inflammatory bowel disease. A recent retrospective study of Somatiic mubtio in HNPC OO-GT Wdhams et a 1,0 long-term neoplasia risk after azathioprine treatment in patients with Crohn's disease and ulcerative colitis (Connell et al, 1994) found a significant 2.5 fold increase in all neoplasms after more than 5 years of azathioprine, and a 17-fold increase in CRC, although this did not reach statistical significance (the absolute number of tumours was small). The same study compared the frequency of CRC in azathioprine-treated ulcerative colitis patients with matched patients who had not received the drug.…”
