Long-term follow-up of surgical intervention pattern in pediatric low-grade gliomas: report from the German SIOP-LGG 2004 cohort

Abstract: OBJECTIVE Neurosurgical treatment is an integral part of the treatment algorithms for pediatric low-grade glioma (LGG), yet patterns of surgical procedures are rarely challenged. The objective of this study was to evaluate surgical treatment patterns in pediatric LGG. METHODS The German Societé Internationale d’Oncologie Pédiatrique (SIOP)–LGG 2004 cohort was analyzed to identify relevant patient and tumor characteristics associated with time to death, next surgery, number of resections, and radiological out… Show more

“…Comparable to the surgical intervention pattern in recent pediatric SML‐LGG cohorts, 15,37 biopsies constituted the majority of tumor‐related interventions in our cohort and complete resections amounted to only 10%. This distribution reflects the eloquence of the SML, impeding the attempt for complete resection, which is only justified when achievable with an acceptable functional outcome 16 .…”

“…Neurosurgery remains the mainstay of treatment with a role for diagnosis, tumor control and relief of mass effect, 9 and technical advances supported substantially an improved surgical management 10‐12 . Complete resections were confirmed as favorable prognostic factor 13‐17 . Key drivers for tumor regrowth or relapse and patterns of behavior of the various tumor subtypes are not fully understood, yet size and site of tumor remnants play a significant role 15,16,18 .…”
