Long-term follow-up of over 1000 patients with salivary gland tumours treated in a single centre

Abstract: Between 1947 and 1992, 1403 patients with 1432 salivary gland tumours were treated at the Christie Hospital, Manchester. There were 1194 epithelial neoplasms: parotid, 1082 (91 per cent); submandibular, 47 (4 per cent); minor glands, 65 (5 per cent). The commonest histological diagnoses were pleomorphic adenoma (n = 776) and adenolymphoma (n = 159). A total of 244 carcinomas were seen (adenoid cystic carcinoma, n = 75). Treatment was primarily surgical, conservative where possible, and determined by tumour ext… Show more

“…Earlier single-center studies that were also based on the 2005 WHO classification have reported these two tumors as being the most common malignant SGTs, but the order differed: one study found an equal frequency of these tumors [24], two studies found ACC to be more common [21,22], and the other two studies, including one from the UK, found mucoepidermoid carcinoma to be more common [9,23]. Before 2005, the frequency of mucoepidermoid carcinoma among British patients was much lower than the worldwide range [1], with a predominance of ACC [10,28]. Again, this demonstrates the changing trends in the incidence of SGT that can be related to a variety of factors, such as institutional referral bias and changing composition of the population.…”

“…Postoperative radiotherapy is indicated when the resection margin is positive or close, 3,4 the tumour is high grade, 3,4 there is tumour infiltration of adjacent organs, or regional metastasis is extensive. 5,6 The prognosis depends on the stage [5][6][7][8] and grade 3,4,9 of the cancer, the completeness of surgical resection, and, when the resection is incomplete, whether or not postoperative radiotherapy is administered. [1][2][3]6,10 Unlike other high-grade salivary gland carcinomas, lymphoepithelioma-like carcinomas (LELCs) are very radiosensitive, [11][12][13] and radiotherapy has been used instead of surgery as the primary treatment of LELC.…”

“…40%-73% of recurrent tumours are multifocal as compared to 0.14%-0.6% in primary untreated pleomorphic adenoma [1] . Renehan et al have reported bone as the most common site for metastases (45%) followed by the head and neck (43%) and lung (36%) [4] . Metastatic deposits have also been discovered in regional lymph nodes, the oral cavity, pharynx, skin, liver, retroperitoneum, kidney, calvarium, and central nervous system [5] .…”
