Long-term follow-up of cervical spondylotic myelopathy treated by canal-expansive laminoplasty

Abstract: We have studied 29 patients for at least five years after canal-expansive laminoplasty for cervical spondylotic myelopathy. The short- and long-term clinical results were compared, and successive radiographs assessed. The long-term clinical outcome was excellent in five patients, good in 12, fair in 11 and poor in one. The mean short- and long-term improvements were 54.4% and 48.5% but patients with focal kyphotic, kyphotic and S-shaped cervical spines did less well, as did those who had a decrease in the inde… Show more

“…Studies involving patients with cervical kyphosis [15°were excluded because this is considered to be a contraindication for posterior surgery by many researchers [20]. Studies involving patients with cervical myelopathy caused by OPLL were excluded because this condition is different from CSM in terms of etiology, pathogenesis, and natural history; hence, this may have affected the surgeon's decision making regarding the surgical approach used.…”

“…Excellent longterm results have been reported using this technique [3][4][5][6][7]. Clinical follow-up studies have shown that laminoplasty generally results in greater patient satisfaction and lower complication rates than laminectomy [8][9][10][11][12].…”

“…Laminoplasty provides spinal cord decompression via canal expansion, thus allowing the cord to migrate posteriorly while preserving posterior bone stock and architecture. Various laminoplasty techniques have been described, and the literature is replete with follow-up studies that have demonstrated better clinical outcomes and patient satisfaction [3,20,21,23,24,26,35,36,37,43,48,50]. Relatively few reports have demonstrated the biomechanical effects laminoplasty has on the cervical spine [4,28,29].…”

“…Laminoplasty eliminates many of these complications and achieves decompression of the spinal cord by allowing the cord to migrate posteriorly away from the compressive lesion, while preserving the posterior bone stock and architecture [22,23]. Clinical follow-up studies have shown that laminoplasty generally results in greater patient satisfaction and lower complication rates than laminectomy [2,18,20,24,26,36,37,49,50].…”
