Long-term Failure Patterns and Survival in a Randomized Dose-escalation Trial for Prostate Cancer. Who Dies of Disease?

Abstract: radiation-induced subcutaneous fibrosis was found in 8 patients (5%) with a slight difference in disfavor of arm B (7%) vs 4% in arm A. Three patients (2%) presented a grade 2 pneumonitis (all in arm A). Overall, quality of life and cosmesis were good to excellent. Conclusions: Concurrent or sequential adjuvant radiohormonotherapy with letrozole is feasible in daily practice. Identifying hypersensitive patients with CD8 RIA or ATM screening will help tailoring treatments and details will be presented at the co… Show more

“…Radiotherapy, especially external beam irradiation therapy (EBRT) is one of the major curative treatment options for localized prostate cancer (CaP) [1]. Several randomized trials have proved benefit of dose escalation to [ 70 Gy with EBRT for localized CaP [2][3][4]. Eade et al [5] recommend doses of [ or = 80 Gy for most men with CaP due to significant benefit for patients' outcome.…”

“…Looking at the long-term results of a dose-escalation trial [63], even in an high-dose arm (78 Gy) a PSA recurrence occurs in 21% of patients in the high-risk group with 8% of local recurrence and 4% of nodal and distant progression. Thus, even with high-dose RT, high-risk disease demands ADT.…”
