Abstract: The smallest incision group had the least surgically induced astigmatism and axial change. All incision groups remained stable and had satisfactory clinical results.

“…As Lever and Dahan observed, 19 OCCIs are self-sealing, pose no added risk, require no extra surgical equipment, and are effective in treating preexisting astigmatism. Clear corneal incisions are invasive and produce a stable flattening effect over the long term, 17 and OCCIs have an enhanced effect. 19 In earlier studies of OCCIs, [19][20][21][22] the mean reduction in corneal astigmatism ranged from 0.50 to 2.06 D. Our study concurs with these observations.…”

“…The astigmatic effect of clear corneal incisions (CCIs) has been put to effective use in modern cataract surgery for controlling preexisting astigmatism with the incision on the steepest meridian. 17,18 Performing an additional CCI opposite the first CCI to enhance the flattening effect has been reported. [19][20][21][22] This method is called paired opposite clear corneal incisions (OCCIs).…”

“…Relaxing incisions, transverse or arcuate keratotomies are also used to correct PEA [17][18][19] but these are more skilled techniques than OCCI, require special instruments, and may have a long-term flattening effect on the cornea [21]. Although others advocate the former because OCCI is a penetrating procedure, and recommend that longer OCCI incisions should be sutured [14], the effect of same-length incision on correcting astigmatism is much greater for OCCI than for arcuate keratotomy [5].…”

“…[1][2][3][4][5][6][7][8][9]20,21 Direct interstudy comparison is difficult because the size of the incision varies between studies, as does the surgical technique. However, the data suggest a significant reduction in SIA in the temporal and BENT (between nine and ten o'clock) location compared with superior incisions.…”
