Long-Term Complications of Native Valve Infective Endocarditis in Non-Addicts

Abstract: Survival after infective endocarditis is fair (81% probability of survival at 10 years), and the most common types of cardiac death are sudden and postoperative. Aortic valve involvement is an independent predictor of the need for late cardiac surgery. The rate of recurrences is not negligible (incidence density at 15 years, 0.0030 per patient-year).

“…Recent experiences in Europe have emphasized the potential for early valve repairs when feasible, especially in patients with anterior mitral valve IE and even in the absence of other conventional indicators for surgical interventions. 220 -222 This approach has a 2-fold potential benefit: Because Ͼ50% of patients with left-sided native valve IE will require valve replacement surgery within 10 to 15 years of the IE episode, 223 this "preemptive" repair strategy has the advantage of earlier-age intervention; and this approach may circumvent ultimate valve replacement requirements, with the attendant risks of long-term anticoagulation.…”

“…This peak may be explained by recurrence of endocarditis in a relatively large group of patients 5 that often requires repeated surgery, especially in prosthetic valve recipients. [15][16][17] The second and third replacement peaks coincide with the identification and publication of risk factors for outlet strut fracture in 1992 and the nationwide mailing of individual risk estimates to all Dutch cardiothoracic centers in 1998, respectively. The consequences of disclosing such important information are now shown empirically.…”
