Long-Term Clinical and Histological Analysis of Integra Dermal Regeneration Template

Abstract: This study showed significant improvements in patient-assessed mobility, softness, and appearance. Collagen and elastic fibrin were present in all specimens, nerve fiber regeneration was limited to the mid- or lower reticular dermis, and adnexal structures were absent. The typical wrinkled appearance of mature Integra reconstruction cannot be said to be entirely due to a lack of elastic fibers.

“…DRT provides an alternative option for wound closure, in the acute setting, in critically ill patients with extensive injuries where otherwise major reconstructive flaps would be considered risky and morbid. Furthermore, DRTs are able to provide long-term functional and cosmetic outcomes with improved skin texture and pliability, results that are comparable to that of autologous skin graft [4][5][6].…”

“…45 Despite many advances in our understanding of wound healing, [45][46][47][48][49] the treatment of full-thickness wounds has focused primarily on regenerating dermis, 50,51 while neglecting or even impeding re-epithelialization of the epidermal layer. No clinically available dressings have all of the characteristics required to repair the dermis and epidermis concurrently.…”

“…52 While synthetic skin grafts (e.g. Endoform TM , Biobrane Õ , and Integra Õ ) are used to enhance dermal healing, 51 these products can take up to three weeks to vascularize, allowing for complications to occur during that extended timeframe. 53 These products also do not directly promote re-epithelialization.…”

“…The advanced wound matrix can expand after application, filling the dead space inside the lesion and absorbing tissue fluids, and may be able to stop inflammation because it does not attract platelets and leukocytes, shifting the host response towards regeneration. The advanced wound matrix eliminates inflammation and related consequences [41].…”
