Background Dermal regeneration template (DRT) has been well implicated in the reconstruction of fullthickness burn injury. This case series specifically presents our experience and our clinical application of Pelnac® to achieve wound closure with complex acute full-thickness defect. Methods A retrospective review of patients treated with Pelnac for complex wound defects from 2008 to 2014 at Concord Burns Unit was carried out. Variables such as wound aetiology, wound size and complications were considered. Results Five patients (four females and one male with a mean age 54±20) all had full-thickness defects (mean defect size 4.3±2.0 % TBSA), some with exposed tendon and bone. The wounds were treated with Pelnac®; the silicone layer was removed at postoperative day 14 and a split-thickness skin graft (0.2 to 0.3 mm) was applied. Clinically, the reconstructed areas demonstrated good granulation tissue at 14 days with good take of the skin graft. There were no major acute graft loss, rejection or associated infection. However, there were small areas of graft loss which did not require regrafting.Conclusions DRT provides a safe and efficacious alternative when dealing with acute contaminated full-thickness wounds. Pelnac® seems versatile in obtaining wound coverage in difficult complex wounds, especially in critically ill patients where free or pedicle flap reconstruction would be problematic. Level of Evidence: Level V, therapeutic study.

Nearly 12 million wounds are treated in emergency departments throughout the United States every year. The limitations of current treatments for complex, full-thickness wounds are the driving force for the development of new wound treatment devices that result in faster healing of both dermal and epidermal tissue. Here, a bilayered, biodegradable hydrogel dressing that uses microarchitecture to guide two key steps in the proliferative phase of wound healing, re-epithelialization, and revascularization, was evaluated in vitro in a cell migration assay and in vivo in a bipedicle ischemic rat wound model. Results indicate that the Sharklet TM -micropatterned apical layer of the dressing increased artificial wound coverage by up to 64%, P ¼ 0.024 in vitro. In vivo evaluation demonstrated that the bilayered dressing construction enhanced overall healing outcomes significantly compared to untreated wounds and that these outcomes were not significantly different from a leading clinically available wound dressing.Collectively, these results demonstrate high potential for this new dressing to effectively accelerate wound healing.Keywords: Micropattern, wound healing, biomaterials, hydrogel, gelatin, microarchitectureExperimental Biology and Medicine 2016; 241: 986-995.

The authors aimed to evaluate the efficacy of an advanced wound matrix (Integra Flowable Wound Matrix, Integra Life Science Corp, Plainsboro, NJ, USA) for treating wounds with irregular geometries versus a wet dressing in sixty patients with diabetic foot ulcers (Grades 3 Wagner ulcer). A randomized clinical trial was conducted in the General Surgery Unit and Geriatric of the Second University of Naples, Italy, in the last 12 months. Forty-seven cases of diabetic foot ulcers were equally and randomly divided into two groups: in a group treated with a wet dressing and expected closure by secondary intention; in another group the lesions were filled with Integra Flowable Wound Matrix and surgical wound edges were either approximated with stitches. The complete healing rate valued at 6 weeks, in the whole study population was 69.56% (Integra Flowable Wound Matrix group, 86.95%, control group, 52.17%; P=0.001). Amputation and re-hospitalization rates were higher in the control group compared to Integra Flowable Wound Matrix group. Therefore, the difference was statistically significant.This new porous matrix, allows a closure for the first intention of the lesion by reducing healing time and the demolition surgery. An advanced wound matrix is not associated with side effects, is well tolerated. Ease of use, absence of adverse effects, and a minimal invasive approach by primary intention closure of the lesion, make it appropriate in the management diabetic foot ulcers.Keywords: Diabetic foot ulcers; Tunneling lesions; Biomaterial; Flowable matrix Short CommentaryPatients with diabetes can develop many different foot complications. Even ordinary injures can get worse and lead to serious complications. Without early and optimal intervention, the wound can rapidly deteriorate, leading to amputation of the affected limb. A diabetic foot ulcer is a critical event in the life of a person with diabetes and is one of the complications of diabetes that can cause life threatening. Diabetic foot ulcers have a major economic impact as well; data have shown diabetic foot ulcers (DFUs) are a major cause of hospitalization for patients with diabetes.
