Long-chain triglycerides improve recovery from myocardial stunning in conscious dogs

Abstract: Exogenous administration of triglycerides during reperfusion of postischaemic myocardium improves functional recovery from stunning. This beneficial effect most likely operates through enhanced FFA availability and/or oxidation since it could be abolished by selective inhibition of the carnitine palmitoyl I enzyme.

“…It has been demonstrated that administration of triglycerides during reperfusion significantly improves myocardial contractility in a canine I/R injury model [27], suggesting that lipid treatment can improve cardiac function. Our hemodynamic data showed that compared with the sham group, the three I/R-treated groups had reductions of all hemodynamic parameters, demonstrating that I/R injury induced damage to cardiac function.…”

“…Baclofen is a relatively weakly lipophilic drug, therefore, a comparatively large dose of ILE may be required to produce a clinical response. It has also been suggested that ILE therapy may be linked to improvements in cardiac function in cases of local anaesthetic toxicoses via a direct effect on the myocardium due to improved fatty acid oxidation (Van de Velde and others 1996). …”

“…A second speculated mechanism is that ILE may increase myocardial calcium levels leading to improved contractility [20,21]. A third possibility is that ILE may improve myocardial ATP synthesis by increasing the amount of available fatty acid substrate [22,23]. A fourth possibility is that some combination of these factors is involved and the beneficial effect of ILE depends upon which effect predominates in a particular scenario.…”
