Lone Atrial Fibrillation Associated with Creatine Monohydrate Supplementation

Abstract: Atrial fibrillation in young patients without structural heart disease is rare. Therefore, when the arrhythmia is present in this population, reversible causes must be identified and resolved. Thyroid disorders, illicit drug or stimulant use, and acute alcohol intoxication are among these causes. We report the case of a 30-year-old Caucasian man who came to the emergency department in atrial fibrillation with rapid ventricular response. His medical history was unremarkable, except for minor fractures of the fi… Show more

“…[147][148][149] A National Institute of Health-sponsored phase-III trial of Cr in the initial stage of Parkinson's disease is currently under way (http://www.parkinsontrial.ninds.nih.gov/). Anecdotal reports have sometimes suggested some possible adverse effects of Cr administration, including ischemic stroke, 150 atrial fibrillation, 151 kidney damage, 152 cholestatic liver injury, 153 rhabdomyolysis, 154,155 and pigmented purpuric dermatosis. 156 However, a cause-and-effect relationship has not been convincingly demonstrated.…”

“…To the best of our knowledge, serious adverse outcomes have only been described in males and only in case reports. These adverse outcomes included renal dysfunction [ 107 , 108 , 109 , 110 , 111 , 112 , 113 , 114 , 115 ], increased lower leg compartment syndrome [ 104 , 116 ], rhabdomyolysis [ 116 , 117 , 118 ], ischaemic stroke [ 119 ], haemorrhagic stroke [ 120 ], liver injury [ 121 , 122 ], atrial fibrillation [ 123 ], acute cholestatic liver injury [ 124 ] and toxic hepatitis [ 122 ]. These events are not replicated in placebo controlled clinical trials [ 125 , 126 , 127 , 128 , 129 ].…”

“…E. The creatine kinase levels suggest acute muscle injury and a muscle, not liver, origin of the elevated AST and ALT. The medical relevance lies in a few case reports, albeit anecdotal, of acute renal failure and arrhythmias secondary to rhabdomyolysis induced by creatine supplementation ( 5,13,15,16 ). The potential for electrolyte disturbances in an aviator has obvious consequences, the worst being a fatal arrhythmia secondary to hyperkalemia.…”
