Localized Pustular Vasculitis in Rheumatoid Arthritis

Abstract

This report describes a case of sterile pustulosis accompanied by underlying leukocytoclastic vasculitis which occurred in the absence of apparent infection or drug allergy on the lower legs of a 46-year-old housewife with 10 years’ duration of rheumatoid arthritis. Histamine-induced vascular changes studied by immunofluorescence microscopy demonstrated a perivascular C3 deposition suggesting the presence of immune complexes. We speculate that some unknown triggers have induced trapping circulating immune complexes of this rheumatoid patient, probably an immunopathogenetic factor of rheumatoid arthritis, with subsequent vascular inflammation producing a localized form of pustular vasculitis.
